Severe hemostatic disorders against the background of COVID-19 have a significant effect on the state of dental status. The study aimed to improve the effectiveness of dental care for patients with COVID-19 disease. To study the dental status of patients with COVID-19 undergoing inpatient treatment, to compare the change in saliva pH with the results of SARS-CoV-2 detection in paired nasopharyngeal smears and saliva samples, to study the microcirculation and oxygenation level of periodontal tissues in patients with moderate and severe COVID-19 undergoing inpatient treatment. 194 people were examined, including 112 patients with the new COVID 19 coronavirus infection and 82 comparison group without concomitant pathology. All patients are divided into five age groups according to their recommendations. The complex of examination included assessment of dental status, determination of saliva pH, the study of microcirculation and oxygenation in the periodontium, and on the phalanx of the index finger of the right hand. Analysis of the results of the study shows that when assessing the dental status, the most characteristic complaints were dryness of the oral cavity, ageusia or hypogeusia, anosmia, or hyposmia. Gum bleeding was observed in two older age groups: in patients aged 45-54 years in 20% of cases, in 55-64 years -18.5% of cases. Less than a quarter of the patients complained of hypersensitivity of the teeth. The study of the saliva pH level revealed normal indicators of the acid-base balance of the oral cavity, which did not depend on the results of the PCR test for COVID-19.
Preoperative correction of periodontal vascular endothelial dysfunction in patients with essential hypertension (HD) can improve the efficiency of dental interventions and reduce the incidence of complications during the implantation process. The periodontal status and functional state of the vascular endothelium were examined in 60 patients with essential hypertension, aged 35 to 45, before and after implantation. The first, main group included 40 people with hypertension, who, in preparation for implantation, along with traditional dental treatment, were prescribed an antioxidant drug Mexidol. Patients of the main group were divided into two subgroups: in subgroup A, a solution of Mexidol was used in the form of applications on the mucous membrane of the gums; in subgroup B, the drug was administered both locally and per os. In the second group, in 20 patients with hypertension, surgical intervention was performed after the traditional complex of therapy. The control group consisted of 10 apparently healthy patients of the same age, with normal blood pressure. Endothelial dysfunction was diagnosed by the level of stable end metabolites of nitric oxide. During the initial examination of patients with hypertension, a significant decrease in the content of nitric oxide metabolites in the blood serum was revealed in comparison with the indicators of patients in the control group. After the course of the therapy, the indicators of nitric oxide in the serum of patients with hypertension increased, more significantly in those who received Mexidol, both locally and orally, which was accompanied by a clear improvement in periodontal status and the absence of complications in the postoperative period.
Chronic generalized periodontitis still occupies a leading position among dental diseases, but the effectiveness of drug therapy is insufficient. One of the promising methods of non-drug treatment is the use of factors of various physical nature, which allows to have a therapeutic effect on various functional systems. The combined use of normobaric hypoxia and a low-energy infrared laser significantly increases the effectiveness of complex treatment of patients with chronic generalized periodontitis. At the same time, the regression of clinical signs of the disease was noted to a greater extent when using laser radiation compared to hypoxic exposure. Taking into account the fundamentally different mechanisms of realization of the biological potential of a low-energy infrared laser and normobaric hypoxia, it can be assumed that the increase in the effectiveness of treatment of patients with chronic generalized periodontitis is due to the additive or potentiating nature of the therapeutic effect when they are combined.
