Diatropics is an evolutionary theory of diversity and the mechanisms of its formation. Identity integration disorder (IID) is a specific mixed form of psychopathological diathesis, characterized by the presence in the personality structure of a combination of symptoms of the three main circles of mental disorders: schizophrenic, epileptic, affective. Our study evaluated the peculiarities of the neurophysiological indices of the brain of persons with IID as consolidating with respect to the main forms of mental disorders, to substantiate the possibility of studying this phenomenon as a diatropic model of mental pathology. A total of 76 subjects with IID and 20 mentally healthy subjects were selected. For neurophysiological examination, cross-correlation analysis of electroencephalography respondents was used. The changes in the connections between the different brain regions of individuals with IID can be described as indicating two main features: 1) a decrease in the degree of interaction between centrally localized structures - the temporal, and upper parietal; 2) an increase in the degree of interaction between the integration of the frontal and occipital regions. The presence of signs of mental disorders in the human population is necessary in order to form a special group of highly intelligent, creatively gifted.
Ulcerative lesions of the cornea and graft in children are a serious problem in pediatric ophthalmology, leading to serious consequences, up to the anatomical death of the eye. The problem of effective treatment of this pathology and corneal tissue regeneration in children remains relevant, despite the large arsenal of drugs. Recently, ultraviolet (UVA) corneal collagen crosslinking has been proposed as an alternative treatment for corneal ulcers in adults. RASK-CXL is the official name used exclusively for crosslinking in cornea inflammatory diseases. In children, corneal collagen crosslinking is still used only for the treatment of keratoconus. We were the first to use this method to treat corneal ulcers and graft ulcers in children.Purpose: to analyze the results of treatment of ulcerative corneal lesions and graft in children using a new device for local UVA crosslinking.Material and methods. For dynamic observation of patients, biomicroscopy with a fluorescein test, photo registration and optical coherence tomography of the eye anterior part were used. The procedure of local UVA crosslinking for the treatment cornea ulcerative lesions was carried out using a new device on the basis of the permission of the Ethics Committee of the National Medical Reseach Center of eye deseases named after Helmholtz of the Ministry of Health of Russia (protocol No. 56 of 21.10.2021).Results. The use of 3–5 procedures of local UVA exposure (against the background of instillations of 0.1 % riboflavin solution) lasting 5–7 minutes with an interval of 1-3 days ensured epithelization of the cornea and the graft and increased visual acuity in children.Conclusion. The first treatment results of corneal ulcers and graft in children using a new portable device for local UVA crosslinking have shown its high therapeutic efficiency and prospects for further use in clinical practice.
The paper proposes theoretical foundations for assessing the integrated efficiency of fishing industry enterprises. A set of parameters is proposed, including factors that characterize the environmental and social efficiency of the enterprise, as well as factors affecting the effectiveness of the enterprise management system. The factors for assessing environmental efficiency included assessments characterizing emissions of greenhouse gases and other harmful substances, the level of energy consumption, as well as the harm caused to water resources. The factors for assessing social effectiveness included assessments related to the level of wages, the level of injuries, the assessment of the effectiveness of labor protection measures and the assessment of staff housing conditions. To determine the effectiveness of corporate governance, factors related to the assessment of the quality management system, the assessment of the quality of risk management systems, and the assessment of the overall financial condition of the enterprise are taken into account. The proposed computer complex consists of two modules. The first module is used to form an integral estimate. The second module is used to correlate the received assessment to one of the seven classes of the state. The proposed provisions are based on the methods of fuzzy set theory and the property of the hierarchical fuzzy inference system, which is based on the Sugeno algorithm. The results of the study led to the conclusion that the system allows you to track the sharp deviation of each of the parameters. The presence of a block of grouping values makes it possible to form classes of states of the evaluated objects. Each of the state classes is identified with one of the seven levels of the ESG score value. The conclusions made open up the possibility of further development of corporate information systems.
Purpose. To analyze the level of endothelin-1 (ET-1) in the tear fluid (TF) of children with primary congenital glaucoma (PCG) and to detect possible correlations between the clinical and the laboratory data. Material and methods. We examined 23 eyes of 15 children with PCG aged between 7 months and 15 years and 3 healthy fellow eyes of these children. 25 eyes of children of same age without glaucoma (somatically healthy with mild hyperopia and/or concomitant strabismus) served as control. A standard ophthalmological examination was supplemented by the flash visual evoked cortical potential, a total and a rhythmic electroretinogram, axial length (AL) echobiometry of the eye, optical coherence tomography of the optic nerve head, enzyme immunoassay (ELISA) of ET-1 level in TF were given. Results. In most children with PCG (95.6 %), the TF level of ET-1 (mean 9.69 ± 3.80 pg/ml) was higher than that in healthy children (4.65 ± 2,02 pg/ml) (p < 0.001). Increased levels of ET-1 (up to 8.90 pg/ml) were also detected in healthy fellow eyes of children with PCG. The mean level of ET-1 in children with PCG aged 3 to 15 years was significantly higher (p< 0.05) lower than in the initial stage. Our study revealed no correlations between the ET-1 level and the degree of retinal nerve fiber layer thinning, or the AL in children with PCG. Conclusion. For the first time, an increased level of ET-1 in the TF in children with PCG was revealed as compared to healthy children. A lower value of ET-1 in the far advanced PCG stage as compared to the initial stage can be explained by long-term chronic eye tissue hypoxia depleting the protective reserves of the eye. Since no obvious correlation between ET-1 level in the TF and clinical and functional data of children with PVG has been found, we cannot definitely view the ET-1 level in the TF as a measure of ischemia degree in PCG. Further studies are needed to obtain reliable correlations.
Purpose: to analyze clinical and functional manifestations and the course of different stages of primary congenital glaucoma (CG) at different times of the disease onset, using the data of admissions to the Department of Eye Pathology in Children of the Helmholtz Research Center of Eye Diseases.Material and methods. 191 patients (320 eyes) with CG, of which 113 were children (191) eyes with primary congenital glaucoma (PCG) aged 1 month to 16 years, underwent standard ophthalmological examination (autorefractometry, visometry, biomicroscopy, ophthalmoscopy), electrophysiological examination (flash VEP, ganzfeld and flicker ERG) and echobiometry (axial eye length measurement).Results. A detailed clinical and functional characteristic of PCG, including structural and functional changes occurring at different stages. Most children with PСG showed advanced stages of the glaucomatous process: in cases where PСG was detected by 1 month of life, the advanced stages claimed 71.1 %, whilst the children diagnosed with PCG between 1 and 12 months of age, the advanced stages were found in 91.2 %. clinical manifestations were found to have variability, not always corresponding to the stages of the disease. In particular, we detected a correlation between disease progression and decreased VEP amplitude (p < 0.05), while no such correlation was found with respet to ERG parameters and VEP latency.Conclusion. Despite the fact that PCG is detected comparative early, the destructive glaucomatous eye damage leads to severe and often irreversible consequences. The timing of CG manifestation is determined by the degree of congenital abnormalities in the eye drainage system that, together with secondary structural changes of eyes (cornea, axial length, retina and ONH) entail a more severe prognosis of PCG in case of an early disease onset. New objective criteria are needed to assess the severity and prognosis of PCG, which is important for the preservation of visual functions, prevention of low vision and blindness.
