From 01.01.2018 year came into force the new order of Ministry of health of the Russian Federation «About carrying out of preventive medical examinations of minors» No. 514н, according to which the dentist is part of the medical Commission and examines children in the following life periods: 1 month, 2 years, 3 years, and then annually until the age of 17. The aim of our study was to compare the prevalence and intensity of dental caries in children, the prevalence of dental anomalies in children aged 2 years, 4 years, 6 years and 7 years, which were carried out preventive examinations in 2016 and 2017. Objective. To assess practicing dentists who treat patients conservatively attitude to antibiotic therapy in general, the degree of awareness of the rules for prescribing antibiotics, approaches to prescribing antibiotics for various dental diseases and therapeutic and preventive manipulations. Materials and Methods. The questionnaire was developed and an anonymous survey-questionnaire of 173 practical dentists conducting therapeutic reception of patients was carried out. The questionnaire included a list of diseases, therapeutic and prophylactic manipulations in which respondents consider the use of antibiotics indicated and questions to assess the respondents’ awareness of modern methods of antibiotic therapy and their views on the rules for prescribing antibiotics. Results. A statistical analysis showed that antibiotics using is considered inappropriate: in the conservative treatment of pulpitis after root canal filling (83.2%), during the conservative treatment of chronic apical periodontitis (66.5%, 95% CI: 59,4-73,5%), in patients with chronic catarrhal gingivitis (69.4%, 95% CI: 62,5-76,2%), in patients with chronic generalized periodontitis mild degree (without exacerbation) (75.1%, 95% CI: 68,7-81,6%), during professional cleaning of teeth (75.1%, 95% CI: 68,7-81,6%). The use of antibiotics is considered obligatory: in the treatment of acute apical periodontitis/exacerbation of chronic periodontitis (with suppuration from root canals) – 37.0% of respondents (95% CI: 29,8-44,2%); ulcerative necrotic gingivitis – 50.3% (95% CI: 42,8-57,7%), with chronic generalized periodontitis in the acute stage – 71.1% (95% DI: 64,3-77,9%), with professional cleaning of teeth in patients with concomitant pathology – 38.2% (95% DI: 30.9-45.4%) Conclusions. Insufficient awareness of practical dentists, leading the conservative reception of patients, about modern methods of antibiotic therapy was revealed. The problems of the current dentistry situation are identified under such conditions of legally approved standards of care and clinical recommendations absence, as well as absence of organizational opportunities to conduct tests to determinate the sensitivity of microflora to antimicrobial drugs before prescribing antibiotics in outpatient dentistry are identified.
BACKGROUND: Maintaining the aesthetic characteristics of direct composite restorations for a long time is relevant. Color stability to food colorants is an important aesthetic characteristic of a composite material. Comparative information about the color stability of modern composite restorative materials, the development of recommendations based on the obtained data aimed at obtaining an optimal clinical result at a mass dental appointment are of interest. AIMS: Comparative study of the color stability of modern composite restorative materials to the most common food colorants. in laboratory conditions. MATERIALS AND METHODS: We studied the color stability of modern restorative materials: Restavrin (Technodent, Russia), GrandioSO (VOCO, Germany), Harmonize (Kerr, USA), Charisma Classic (Kulzer, Germany), Filtek Z250 (3M, USA/Germany), Estelite Asteria (Tokuyama, Japan). Black coffee, black tea, carbonated soft drink cola, dry red wine and cognac were used as coloring solutions. Exposure - 14 days, temperature - 37C. The control solution - distilled water. Vita Easyshade Advance 4.0. spectrophotometer was used. The average deviation of the color of the studied samples from the reference indicators (∆E), the total color stability of the studied materials ( ∆Em) and the coloring potential of different coloring solutions in relation to composite materials ( ∆Ec) were determined. RESULTS: Data were obtained on the deviation of the color of the studied samples of composite materials from the reference indicators (∆Е) depending on the coloring solution. The total color stability of the studied materials in relation to food colorants ( ∆Em) as this characteristic deteriorates: Estelite Asteria - 8.640.08, Harmonize - 10.300.14, Restavrin - 12.300.12, GrandioSO - 12.960.10, Charisma Classic - 13.940.14, Filtek Z250 15.820.15. The most aggressive food colorants with respect to composite restorative materials were black coffee ( ∆Ec = 20.080.12), dry red wine ( ∆Ec = 19.180.10), black tea ( ∆Ec = 18.540.15). CONCLUSIONS: Information about the color stability of composite restorative materials to various food colorants should be taken into account when planning the aesthetic restoration of teeth located in an aesthetically significant area, taking into account the individual nutritional preferences of patients.
