A 54-year-old female underwent uneventful phacoemulsification cataract surgery in the left eye. On postoperative day 1, she developed sudden visual loss. The examination was unremarkable except for a relative afferent pupillary defect. Neuroimaging revealed features suggestive of pituitary macroadenoma complicated by pituitary apoplexy (PA) causing optic chiasmal compression. Surgical intervention was done and the other eye vision was salvaged. This is the first case report documenting cataract surgery as a precipitant of PA, resulting in sudden visual loss.
