Background: Hemarthrosis is a common clinical presentation of patients with severe and moderately severe hemophilia. Severe pain, swelling, and loss of function involving knee, ankle, elbow, and shoulder joints are commonly seen. In India, except for paracetamol and some non-steroidal anti-inflammatory drugs (NSAIDs), opiate analgesics are not easily available even in the mainstay of treatment; i.e., factor concentrates are also not available regularly. Hence, there is an unmet need for exploring alternative management strategies in this condition in India. Objective: To assess the effect of homeopathic medicines on pain and acute hemarthrosis in hemophilia when factor concentrates are not available and paracetamol in adequate doses proves inadequate. Patients: 343 patients with hemophilia (PWH) from Nashik, Mumbai, and Surat presenting with hemarthrosis were prescribed homeopathic medicines in addition to paracetamol and RICE (rest, immobilization, cold application, and elevation). They were assessed using standard techniques. Results: 1,679 episodes of hemarthrosis in major joints were encountered between December 2007 and March 2014, in 343 patients. In 1,580 of the 1,679 hemarthrosis episodes (94.1%), bleeding/inflammation was arrested and pain relieved with homeopathic medication. Additional factor concentrate was required in 99 patients (4.48%). The mean pain score improved from 6.88 ± 2.118 to 1.5 ± 0.34 over 6-24 h following the homeopathic medicines (p < 0.0001). The swellings were also substantially reduced (p < 0.001). The number of joint bleeds per month was reduced significantly under the influence of therapy (p < 0.0001), showing the long-term disease-modifying effect of the treatment. Conclusion: Homeopathic medicines without factor concentrates appeared to reduce bleeding and pain in PWH presenting with hemarthrosis and could have influenced the long-term frequency of bleeding.
Introduction and background:Haemophilia is an X-linked inherited genetic disorder that impairs the body's ability to make blood clots. It results in spontaneous uncontrolled bleeding. Maximum of its manifestation are acute and require immediate intervention. Therefore this study is aimed at focusing on impact of telemedicine in emergency management of haemophilia in remote areas. Materials and methods: At Multicentric HIH centres the patients are prescribed with homoeopathic medicines as per the symptom similarity which is the base for homoeopathic treatment, and given constitution medicines in Fleiss kappa manner, on monthly basis. Patients are provided with emergency kits which consist of around 30 medicines which have been formulated considering haemorrhagic diathesis, and articulation problems, oedema, ecchymosis, pain, inflammation, bleeding, tenderness etc. This tool employed in telemedicine in remote areas to manage acute conditions on the proper time.Result: Total number of interactions of these patients with a team of HIH is 22926, out of which 43.59% was telephonically managed emergencies. In Thane, Surat and Amravati, around 75% of interactions are dealt with the use of social media that is17722 emergencies have been managed telephonically. Conclusion:Telemedicine has efficiently helped to manage and control acute episodes of people with haemophilia and thereby increasing their quality of life as well as the quality of healthcare with homoeopathic medicines.

This paper addresses the challenges faced by individuals with visual impairments when shopping online and proposes a solution to make ecommerce platforms more accessible for them. In India, most non-governmental websites are not accessible to visually impaired individuals, which limits their autonomy and requires them to seek assistance from others. To address this issue, we propose an ecommerce application that utilizes speech recognition technology to search for products. The proposed system architecture includes the use of the Web Speech API for speech recognition and a web interface that provides audio instructions for navigation. This approach offers visually impaired individuals a more convenient and flexible way to access ecommerce platforms, thereby promoting their independence and inclusivity in society. The proposed system aims to make ecommerce accessible to all individuals, regardless of their capabilities, social status, or purchasing power.
