The evolution of women with ASCUS PAP smear must be done in an orderly fashion. The family doctor must communicate with the gynecologist following standardized procedures so that the lesions can be discovered as early as possible. For women with ASCUS PAP smear it is recommended to take another cytological examination in 6 months and also test themselves for HPV. After said investigations, if need be, a colposcopy is also recommended. Women witch are HPV negative repeat the PAP smear yearly. Patients presenting ASCUS PAP smear and are HPV positive will be investigated as if they had LGSIL PAP smear. In this case a colposcopy will be done. If the cytology exam result is a CIN 1 type, the exam will be repeated early along with the HPV exam. If it’s a CIN 2 or CIN 3 type, the patient will be investigated every 4 or 6 months for a period of 2 years. The HPV test will be made at least once a year. Women with 3 negative result are tested once every 3 years.
In our study, we followed young women with ASC-US and LG-SIL positive smear to confirm we used the correct treatment and to avoid excessive treatment. The study was made on women between 20 and 30 years. They were further divided in 2 groups: one between 20 and 25 and another between 25 and 30. From the data analysis we concluded: • ASC-US are 2 times more often than LG-SIL in the 20-25 year old group; • In the 25-30 year old group results showed a 1:1 ratio between ASC-US and LG-SIL. • Regarding HPV testing the infection is twice as often found in the 25-30 year old group than the 20-25 year old group. • In the younger group 74,46% are infected with HPV. In this group, the association of HPV with ASC-US is found in 54,2% of the cases while only 20,2% of the cases associate HPV and LG-SIL. We can conclude that lesser cytological modifications are usually associated with HPV infection in the 20-25 year old group. • In the older group 73,95% are infected with HPV. In this group the association of HPV with ASC-US is found in 27,9% of the cases while only 46,04% of the cases associate HPV and LG-SIL. We can conclude that in the 20-25 year old group HVP infection is usually associated with ASC-US 16,5% of all the cases The 25-30 year old group mostly associates HPV infection with LG-SIL, 32,03% of all the cases. Conclusion: HPV infection is found mostly associated with ASC-US in the 20-25 year old group while in the 25-30 year old group it is mostly associated with LG-SIL.
