Purpose To study midterm efficacy and safety of combined Visco-Trab operation for management of advanced glaucoma. Methods 168 eyes of 148 patients with advanced glaucoma had Visco-Trab operation (a merge of both viscocanalostomy and trabeculectomy operations). Mean follow-up was 29.1 ± 22.2 months. Criteria of success were intraocular pressure (IOP) of 14 mmHg or less with or without glaucoma medications, with no devastating complications, loss of light perception, or additional glaucoma surgery. Results IOP, number of glaucoma drops, and visual field mean deviation were significantly reduced (11.9 ± 5.6 mmHg, 0.7 ± 1.2, and 14.2 ± 6.3 dB, compared to preoperative values of 24.4 ± 9.9 mmHg, 2.8 ± 1.4, and 17.3 ± 6.3 dB, respectively). Success was reported in 136 of 168 eyes (81%) without (100 eyes, 59.5%) or with (36 eyes, 21.5%) glaucoma medications. A functioning bleb was seen in 2/3rd of eyes; diffuse (59 eyes, 35%) and thin ischemic (54 eyes, 32%). Predictors for failure to achieve the target IOP included previous ocular (p = 0.01) or glaucoma (p = 0.04) surgery, number of preoperative glaucoma medications (p = 0.029), and severity of glaucoma (p = 0.058). Conclusion Combined Visco-Trab operation proved safe and effective, on midterm follow-up, in reducing IOP to the proposed target level in eyes with severe glaucoma via enhancing internal and external filtration.
The Goods and Services Tax (GST) implementation in Malaysia has received mixed reactions from various parties such as businesses, policy makers, professionals, business associations and individuals. Despite the rejections from these parties, the government proceeded with implementing GST. On consequence, this has led to GST compliance issues. One of the issues that have been highlighted is high compliance cost. To address this issue, various Government Support Programs (GSPs) have been implemented in Malaysia particularly financial and credit assistance. Therefore, this study aims to determine the effect of GSPs on GST compliance among batik manufacturers in Malaysia. In addition, this study also examines the relationship between compliance cost in terms of fiscal cost, time cost and psychological costs on GST compliance among Malaysian batik manufacturers. Data was collected through a structured survey among Malaysian batik manufacturers. Based on the finding, the result showed that GSPs, time cost and psychological cost do affect GST compliance while fiscal cost did not affect GST compliance among batik manufacturer. The findings in this study provide awareness to the government on the need of more GSPs and other incentives to the batik manufacturers in order to encourage them to comply with GST.
Computed Tomography -Clinical Applications 294 2. PET technique -Basic and advance 2.1 Basic PET technique PET is a nuclear imaging technique which detects abnormal tissues with the help of molecular imaging probe like flurodeoxyglucose (FDG), dopa, fluoroethylcholine (FLT) and fluoroethylthymidine (FET). These probes can be tagged with radioisotopes like 18-Fluorine ( 18 F) and carbon ( 11 C , 12 C). These unstable isotopes while emitting beta plus (β + ) rays will enable the PET camera to localize the highest decaying activity in the body. The process can be captured and image. However, due to poor resolution of PET camera, a standalone PET system yields poor lesion localization. To improve the resolution, transmission from Germanium-68 isotope was utilized for attenuation correction but the technique is time consuming. All new PET systems are now improved by incorporation with a CT system. The CT parameters are used for attenuation correction of PET images. The new integrated system is capable in delivering higher quality images with improved resolution (Figure 1) . The time spent for each examination is shorter in comparison to using the conventional old PET system.
