Olgular›n 78'i (%38) erkek, 127'si (%62) kad›n olup 103'ü (%50.2) çocuktu. Çocuklar›n en küçü¤ü 5, en büyü¤ü ise 18 yafl›nda olup, yafl ortalamas› 14.03±3.56 olarak belirlendi. Çocuk olgular›n 48'i (%46.6) erkek, 55'i (%53.4) k›z idi. 42 çocukta (%40.8) travma sonras› stres bozuklu¤u tespit edildi. 98 çocu-¤un (%95.1) istismar zanl›lar› yabanc›lard›. Okul ça¤›na gelmifl olan 24 çocuk (%23.3) okuma-yazma bilmemekteydi. ‹stismara u¤rayan 26 çocukta (%25.2) ayn› zamanda darp-cebir izleri de görüldü, böylelikle cinsel istismarla birlikte fiziksel istismara da maruz kald›klar› belirlendi. Çocuk olgular›n 16's›n›n (%15.5) hafif mental retarde olduklar› belirlendi. 7'si erkek (%43.8), 9'u (%56.2) kad›n olan mental retarde çocuklar›n yafl ortalamas› 16.25±2.543 olarak hesapland› ve en küçü¤ünün 11, en büyü¤ünün ise 18 yafl›nda oldu¤u belirlendi. Sadece bir mental retarde çocukta travma sonras› stres bozuklu¤u tespit edildi. Mental retarde çocuklar›n 6's›nda (%37.5) fiziksel istismar da tespit edildi. Mental retarde çocuklar›n tamam›n›n yabanc›lar taraf›ndan istismar edildi¤i tespit edildi.Cinsel istismarlarda çok çeflitli etmenler söz konusu olsa da istismara u¤rayanlar aras›nda mental retardasyonlu çocuk olgular›n da önemli bir yer tuttu¤u, seksüel sald›r›lara karfl› bu olgular›n daha korumas›z kald›¤›, bu olgulara yönelik seçici ve spesifik e¤itim programlar› ile t›bbi ve sosyal olarak destek olunmas› gerekti¤i sonucuna var›ld›.Anahtar kelimeler: Cinsel istismar, fiziksel istismar, ço-cuk, mental retardasyon SUMMARYIn sexual abuse several factors are present, we have aimed to take an attention the density and distribution of adult-child and mental retarded sexual abuse victims in this study. 205 sexual abuse cases which have applied to our department between January 2000 and March 2008 have been evaluated by being taken into study scope.78 of the cases (38%) were male and 127 were female (62%). 103 of the cases were children (50.2%). Youngest of the children were 5 years old and oldest was 18 years old, with the average age of the children calculated as 14.03±3.56. 48 of the children cases were male (46.6%) and 55 were female (53.4%). In 42 children (40.8%) post -traumatic stress disorder was detected. The accused of 98 children (95.1%) were strangers. 24 children (23.3 %) who have come to education were illiterate. At 26 of the cases (25.2%), physical violence and compulsion traces were also seen, thus they were concluded to have been exposed to physical abuse as well as sexual abuse. 16 of the children cases (15.5%) were detected to have slight mental retardation.
Abstract:Foreign body ingestion is a common event that mostly occurs accidentally in the pediatric population. In adults, unintentional swallowing of objects occurs primarily in the elderly population, alcohol abusers and persons with learning disabilities, whereas intentional episodes occur in individuals with mental disorders and prisoners. Foreign objects commonly ingested by children include coins, magnets, toy parts and button batteries while in adults swallowing of fish bones, chicken bones, needles and dentures are common. Ingestion of razor blades, other sharp metallic objects and batteries was reported among prisoners as a means of self-harm. Ingestion of multiple AA batteries by adults usually occurs as a result of personality disorder or psychiatric illness, whereas it may be used by inmates to draw attention or as means to get away from the prison environment. Repeated foreign body ingestion as mode of self-harm poses challenges for hospitals by creating increased workload, for prison staff due to extra working hours and liabilities and for countries as a result of additional costs incurred by treatment of such prisoners. In the present study, we aimed to draw attention to the need to take measures to prevent such cases and to offer well-designed psychological support programs to prisoners in the future by discussing a case of multiple foreign body ingestion in a 37-year-old incarcerated male charged with prison sentence of 22 years who swallowed an unprecedented high number of AA batteries (nine).
