Abstract:Objective: To evaluate the accuracy of fine needle aspiration cytology (FNAC) in comparison to paraffin section in thyroid swelling. Study design: Cross sectional study. Place and duration of study: Department of Surgery, Bangabandhu Sheikh Mujib Medical University from July 2009 to June 2011. Patient and method: One hundred patients with enlarged thyroid gland of both sexes were selected from admitted patient of Surgery department. All patients had preoperative FNAC, performed by pathologist at the Histopathology department and postoperative specimen were also examined and histopathological diagnosis were made. All FNAC diagnosis were correlated with the histopathological diagnosis. Results: Out of 100 patients 23 were male and 77 were female, male-female ratio 1: 3.3. In FNAC 76 patients were diagnosed as benign lesion of which 8 were later diagnosed as malignant on histopathological examination (68 were true negative 8 were false negative). 22 cases were diagnosed as malignant, both on FNAC and histopathological examination. Only 2 cases were diagnosed as follicular neoplasm on FNAC but on histopathological examination diagnosed as follicular carcinoma (22 cases were true positive) in this study. Overall sensitivity of FNAC were 68.75%, specificity 100.00% and accuracy 90%; PPV=100% & NPV=87.18% Conclusion: FNAC is a reliable, safe and accurate method as a preoperative evaluation in thyroid gland swelling before surgery. FNAC has more accuracy and specificity in detecting thyroid gland malignancy and therefore it is a reliable diagnostic test for evaluating thyroid swellings.
Introduction: The aim of this study was to screening the Sensitivity of Alvarado score among the hospitalized suspected appendicitis patients at Dhaka National Medical College Hospital for determine the diagnostic accuracy. Materials and Methods: It was an observational type of descriptive study, conducted in the Dhaka National Medical College Hospital, Dhaka, during the study period of July 2015 to December 2015. The study was approved by the institutional ethical committee. Results: Most of the appendicitis patients belonged to the between 21-30 years which was 64 (32%). Male appendicitis patients (52%) are more than the female patients (48%). Majority of the patients (69%) complains pain occurs in the Right iliac fossa. The sensitivity of Alvarado scores was 81.60%, specificity 74.58%, accuracy 79.35%, positive and negative predictive values were 87.18% and 65.67% respectively. Objectives: To find out the efficacy of single dose antibiotic vs multiple dosesin preventing wound infection following appendicectomy for uncomplicated appendicitis. Methods: A prospective comparative study was conducted in the Department of Surgery, Chittagong Medical college Hospital from January'2009 to June'2009. 100 patients with uncomplicated appendicitis who underwent appendicectomy during that period were included in this study. Among them 50 cases were included in study group ·(SG) who were given only single dose of combination drugs ( lnj. Cefuroxime + lnj. Metronidazole).ln rest of the 50 cases (CG), antibiotics were continued for 7 days postoperatively. Result: Maximum incidence of acute appendicitis was in the 2nd and 3rd decade of life with male preponderance .Rate of wound infection in the study group and control group was 4% and 2% respectively which was not statistically significant .The duration of antibiotic therapy had no significant effect on the length of hospital stay between the two groups ; 2.48 days vs 2.9 days [mean + s.d. 69.6 # 16.8 hours ] in the study and control group respectively, though treatment cost was higher in control group in comparison to the study group . Conclusion: Single dose of preoperative antibiotics is adequate for prevention of postoperative wound infection following appendicectomy for uncomplicated appendicitis. Objective: To determine the complications of the patients undergoing total thyroidectomy. Method: A prospective study was held at Department of Endocrine Surgery in BSMMU from January 2006 to December 2006 Method: Fifty patients with multinodular goiter and carcinoma thyroid of both sexes were selected from admitted patient of endocrine surgery department. All patients were evaluated preoperatively and total thyroidectomy was done and post operative was followed up. Out of 50 patients 35 patients were female and 15 patients male, male - female ratio 1 :2.3. 40 patients were carcinoma thyroid, 10 patients were multinodular goiter. The overall incidence of postoperative complication in this series was 4. out of 50 patients 1 patient developed haematoma. 1 patient developed wound infection and 1 patient developed transient tetany on 2nd postoperative day which was improved later on. 1 patient developed a thyroid nodule 6th month after total thyroidectomy for carcinoma thyroid. In long term from total thyroidectomy 40 patients were followed up, 10 patients did not attend an subsequent follow up. Out of 40 patients 36 patients were out of complication. Conclusion: The results are comparable with the current published data and demonstrate that total thyroidectomy can performed with minimum complication rate.
