We reviewed the outcome of 33 patients (45 feet) treated by scarf osteotomy for hallux valgus deformity with an intermetatarsal (IM) angle equal to or greater than 16°. The average follow-up was 26 months. The hallux valgus angle improved by an average of 21.1°from a preoperative mean value of 32.1°. The IM 1-2 angle improved an average of 9.9°from a preoperative mean value of 18.3°. American Orthopedic Foot and Ankle Society (AOFAS) score changed from a preoperative average of 35.7 points to 89.8 at follow-up. Our midterm results indicate that the scarf osteotomy provides an effective method for the treatment of severe bunion deformity.Résumé Nous avons examiné le résultat de 33 malades (45 pieds) traité par ostéotomie scarf pour hallux valgus avec un angle intermétatarsien égal ou supérieur à 16°. La suivi moyen était 26 mois. L'angle de l'hallux valgus a été amélioré avec une moyenne de 21.1°après l'opération à partir d'une valeur préopératoire de 32.1°. L'angle intermetatarsienne 1-2 avait une moyenne de 9.90 à partir d'une d'une valeur préopératoire de 18.3°. Le score AOFAS moyen a progressé de 35.7 points à 89.8 points au recul.
