El Nino 2015 has made a prolonged drought. This phenomenon has influenced the attitudes and perspectives of the people towards nature. This study aims to describe the change in natural management at the post-drought. El Nino causing the debit of five water springs decreased and dried. This condition trigered to conflicts of interest and ownership of the spring betwen Laluhe and Marampit Villages. The community rise its awareness to the importance of trees around the spring so that they tried to replant tress and to limit the use of natural resources and provide a village regulation in 2012 on the good governance of natural resources. In 2017, five other villages on Marampit Island agreed to support the efforts of tree planting and protection of springs done by Marampit villages. The Marampit community can be catagorized as a higher perspective and care of its nature resource than the others. In fact, the changing in natural resource management is more influenced by culture than other social factors. This social capital is an opportunity to improve environmental conditions to be more stable.
Rare coexistence of disease or pathology Background:Tuberculosis (TB) is the leading infectious cause of mortality in developing countries. In pregnancy, TB may cause complications including intrauterine growth restriction (IUGR) and vertical transmission of TB. Recurrent TB infection can cause permanent damage to lung tissue, and formation lung bullae can occur. Giant lung bullae may be harmless in normal conditions, but cause concern if the patient is pregnant.
Case Report:A 22-year-old G1P0A0 woman at 9 months of pregnancy came to our institution due to labor pain. She also presented with active TB from 1 month before admission. Chest X-ray findings suggestive of tuberculosis and giant bullae were found. She was diagnosed with active 1 st phase of labor with active TB and giant lung bullae. The Cardiothoracic Surgery Department was on standby during labor to take countermeasures if bullae rupture occurred. The 2 nd stage of labor was shortened with vacuum extraction. After delivery, the patient was shifted to an isolation room and started on antitubercular drugs.
Conclusions:Tuberculosis in pregnancy leads to many adverse outcomes, both in maternal and fetal. Prolong TB infection also causes lung scaring, which leads to formation of bullae. Possible causes of rupture of bullae during pregnancy are Valsalva maneuver during labor, increased alveolar ventilation, and positive-pressure ventilation during general anesthesia. Normal vaginal delivery would increase the need for Valsalva maneuver, and vacuum extraction is advised to shorten the 2 nd stage of labor to minimize the need of Valsalva maneuver and avoid increased intrapulmonal pressure.
The number of Floating Net Cages (FNC) in Cirata reservoir has exploded four times more than allowed. It makes water quality problem severe because 22.260 tons/month of stools and excessive feeding threaten the benefits, functions, and service life of the reservoir. This is a review paper to explore FNC waste management methods, the potential of integrated FNC management, and the implementation strategies. Twenty-eight articles published between 2015 and 2019 abaout aquaculture technology have been discussed to improve understanding of FNC waste management. Several solutions have been recommended based on an integrated management approach. Technical ideas are proposed for water control and in situ treatment management. Management of water control reduces the concentration of pollutants by increasing water destratification. The IFCAS (Integrated Floating Cage Aquageoponics System) - PFF (Precision Fish Farming) - IMTA (Integrated Multi-Trophic Aquaculture) concept is an effort to manage FNC using both approaches. The IFCAS-IMTA concept can reduce the main concept of waste by creating the nitrification and assimilation of phosphate. The concept of PFF can improve the capacity of farmers in water quality detection and monitoring. The manager needs to modify aquaculture techniques in water and nutrition, feeding formulations, technological innovations, and the inclusiveness of scientific-based management. Management must involve FNC farmers, research and development institutions, Cirata authorities, Fisheries and Marine Service of West Java Province, and investors.
