Comparison between the rotary (Hyflex EDM ® ) and manual (k-file) technique for instrumentation of primary molars: a 12-month randomized clinical follow-up study* Rotational instrumentation is an alternative for the clinical practice of pediatric dentists. However, there are few records in the literature on the clinical and radiographic aspects of treated teeth over time. Objective:Compare instrumentation time and filling quality between manual (k-file) and rotary (Hyflex EDM ® ) files, and clinically and radiographically follow-up the treated teeth for 12 months. Moreover, the characteristics of glass ionomer restorations and their interference in the treatment prognosis over time were evaluated. Methodology: In total, 40 children with pulp involvement in primary molars received treatment with Hyflex EDM ® or manual rotary files, performed by an operator. Clinical and radiographic aspects were observed at different times to determine the effectiveness of each technique. Results:The rotary system reduced instrumentation time when compared to the use of manual files (p≤0.05), but there was no difference in filling quality between the groups (p≥0.05). Moreover, both types of instrumentation were effective for 12 months (p≥0.05), and restoration retention influenced the emergence of periapical lesions (p≤0.05). Conclusion: Although rotary files reduce clinical time, the clinical and radiographic aspects of both techniques were similar over 12 months. Moreover, restoration retention has been shown to be related to treatment prognosis.
INTRODUÇÃO: o câncer de cabeça e pescoço é a denominação para o conjunto de tumores envolvendo cavidade oral, faringe e laringe. Os principais fatores de risco são tabaco e bebidas alcoólicas, quando combinados potencializam o desenvolvimento da doença. O ideal é realizar o atendimento odontológico prévio no máximo 2 ou 3 semanas antes do início do tratamento oncológico para eliminar infecção, elementos retentivos, reduzir a atividade microbiana, além de esclarecer as orientações de higiene bucal ao paciente. OBJETIVO: este relato de caso tem como objetivo apresentar a importância da manutenção da saúde bucal de um paciente oncológico prévio ao tratamento de câncer. METODOLOGIA: a coleta dos dados se deu na UAPS através do PEC e-SUS e das consultas odontológicas realizadas por uma cirurgiã-dentista da Equipe de Estratégia da Saúde da Família, vinculada ao Programa Estadual de Qualificação da Atenção Primária à Saúde (Qualifica-APS) -ES. O atendimento odontológico foi sugerido pelo oncologista responsável pelo caso, o paciente apresenta câncer de laringe. Portanto foram realizadas 5 consultas odontológicas durante o segundo semestre de 2022, visto que o paciente apresentava periodontite crônica, além de ser ex fumante e elitista. Foi executado raspagem supragengival manual da arcada superior e inferior, restauração classe II extensa com cimento de ionômero de vidro, exodontia da raiz residual do dente 36. Ressalta-se a complexidade clínica, e a dificuldade durante o atendimento, devido à debilidade do paciente visto que o mesmo se encontrava com traqueostomia. RESULTADOS: Após 2 meses de tratamento, foi alcançado a adequação do meio bucal, entretanto foi explicado ao paciente a importância das consultas de retorno a cada 3 meses para acompanhamento e que durante seu tratamento outras alterações poderiam surgir, como xerostomia, queilite angular, mucosite, cárie por radiação, devendo o mesmo retornar com urgência à UAPS. CONCLUSÃO: Sendo assim, podemos concluir que o tratamento do câncer se dá de maneira multidisciplinar e devemos destacar a importância da atuação do cirurgião-dentista que contribui com promoção de saúde com ênfase na prevenção, detecção e cuidado frente as complicações orais de pacientes oncológicos antes, durante e após o tratamento de câncer, proporcionando uma melhora na qualidade de vida.Palavras-chave: Cirurgião-dentista, Neoplasias de cabeça e pescoço, Equipe multiprofissional, Estratégia saúde da família (esf), Manifestações bucais.
Introduction: Despite advances in caries treatment, the disease still afflicts populations deprived of access to goods and services. Objective: To determine the prevalence of dental caries as well as the dental treatment needs for children aged five to twelve years old from rural public schools in Alfenas/MG. Material and method: Cross-sectional descriptive observational study, carried out through an epidemiological survey, in which data were collected through clinical examination of 364 children from five rural schools in the city. These tests were performed by a calibrated examiner (intra-observer kappa = 0.93), within the institutions, after signing the Free and Informed Consent Form by the person in charge. The prevalence of caries and the need for dental evaluation were observed, using the DMFT and DMFT-D according to the codes and criteria recommended by the WHO. For data collection and analysis, they were processed in the EPIBUCO program, version 2004. Result: At 5 years of age, there was a prevalence of caries in 70% of the children, dmft of 2.98 and a mean of 2.13 deciduous trees in need of restorative intervention, corresponding to 71.44% of dmft. At 12 years of age, a DMFT of 2.20 and a mean of 0.8 permanent teeth in need of restorative intervention per individual were reported, which represents 36.44% of the index. Conclusion: The results show the high prevalence of caries in children from rural schools, thus showing a worse condition than that observed in the Brazilian population, being also below the O.M.S. goals.
