Identification of the area of services provided by the clinical pharmacist in patient care after kidney transplantation • Kidney transplant is a life-extending treatment of choice in end-stage renal disease. Until November 2018, 805 kidney transplantations from deceased donors were performed in Poland and the waiting list for the kidney transplantation procedure is 1176 patients. Optimal care of the patient focuses on: prevention of rejection and organ loss after transplantation, optimization of immunosuppressive therapy, reduction of side effects and toxicity of medication, prolonging long-term survival, as well as providing more cost-effective therapy and cooperation with the patient to improve compliance and medical adherence. This work identifies and describes the area of services provided by the clinical pharmacist (e.g. medication management, cooperation with the transplant team, patient education) as part of patient care after transplantation. The comprehensive goal of the offered pharmaceutical services is to provide high-quality patient care and increase the effectiveness of pharmacotherapy.
