Schwannoma (neurilemmoma) is a benign tumour that arises from the Schwann cells which cover the nerve sheaths of the peripheral or autonomic nervous system especially the sympathetic system. Reported cases of schwannoma arising from the sinonasal tract are rare. We report a case of a 28-year-old man who presented to otolaryngology-head and neck surgery clinic with left cheek swelling and left-sided nasal obstruction for 1-year period. Endoscopic examination and high-resolution CT scan revealed a homogenous expansile mass occupying the left maxillary antrum extending to the left orbit and left ethmoidal air cells. The mass was excised through endoscopic endonasal approach followed by Caldwell-Luc approach for the residual tumour. Postoperative histopathological examination revealed benign type Antoni A schwannoma.
Langerhans cell histiocytosis (LCH) is a rare condition that presents clinically in various ways. The cause and subsequent development of LCH are idiopathic and not fully understood. This disease is mainly seen in childhood. It is rare to have bilateral temporal bone LCH as the initial presentation. LCH can affect many organs. However, the bilateral involvement of the temporal bone is very uncommon. Therefore, we believe documenting cases of this presentation can lead to a better understanding of the epidemiology and prevalence of the disease, which can contribute to its management planning. A one-year-old boy was referred to a tertiary otolaryngology clinic with bilateral postauricular swelling, hearing loss, but no tenderness or ear discharge. During the patient evaluation, a CT scan was requested to further investigate the bilateral swelling, which showed bilateral bony destructive lesions in the temporal bone area. Next, the patient was scheduled for a biopsy of this lesion under general anesthesia. A biopsy of the right mastoid confirmed the diagnosis of LCH. The patient was started on LCH IV protocol for multifocal bone lesions (MFB) with special site induction. A follow-up fluorodeoxyglucose positron emission tomography/CT (FDG PET/CT) was performed on the whole body with the impression of mild interval improvement of the temporal bones’ masses bilaterally with stable bilateral cervical lymphadenopathy. LCH is a rare pathology that requires comprehensive effort from various medical and surgical teams to reach the right diagnoses and start the patient on the best available treatment plan.
Background: Halitosis is a significant change in breath odour that has a negative social impact, affecting many people around the world. It can cause social and personal isolation, decrease the quality of life and may indicate the presence of a more severe disease.Objectives: This study was conducted to assess the knowledge and awareness levels of the causes of halitosis among Taibah University students in AL-Medinah city, Saudi Arabia.Methods: A self-administered electronic questionnaire on knowledge and awareness of the causes of halitosis was distributed on a randomly selected sample of students from Taibah University. Results:The response rate was 45% (540). Our questionnaire revealed that most respondents thought halitosis was normal after eating strong-smelling food (92%), sleeping (72%) and smoking (71%). Ninety-seven percent of the participants said that oral diseases are the most common causes, while only 40% considered the stomach to be the most frequent pathological cause. Great percentages (<55%) related nine causes of bad breath (out of the given 15) to halitosis. According to 315 (58%) respondents, dentists are the most qualified specialists to treat halitosis and only 53 (9%) thought that halitosis patients should be referred to ENT specialists. Finally, more than half the population said they had not received any education about halitosis. Conclusion:This finding demonstrates that knowledge and awareness about halitosis causes seem to be insufficient among Taibah University students. Therefore, health officials must increase their efforts in educating the society about this serious problem.
