Chylothorax is an abnormal collection of milky white fluid called chyle in pleural space, and is an uncommon cause of respiratory distress in neonates. There is high morbidity and mortality, if not managed appropriately. Here we report a rare case of congenital bilateral chylothorax presented with respiratory distress in our NICU on day one of life. Thoracocentes revealed chyle on both the sides. In view of high drainage of chylous fluid, besides TPN, Octreotide infusion started and the baby responded well.
Mediastinal benign teratoma is a rare primary germ cell neoplasm composed of well-differentiated tissues derived from more than one of the three embryonic germ cell layers. Most of the mediastinal teratomas are found in the anterior mediastinum. The posterior mediastinal teratomas are rare. We describe a rare case of an infant with benign cystic teratoma in the posterior mediastinum presenting with recurrent respiratory distress, successfully treated by surgical excision with no recurrence in a 1-year follow-up.
Inguinal abscess is uncommon in infants and children. Majority of cases in the infantile period is primary and they present as leg or groin swelling, limitation of limb movement and pain. The source of microorganism is from the haematogenous spread of normal flora of the skin due to immature immune system. Ultrasound is the investigation of choice which confirms the diagnosis and delineates the areas of collection. Antibiotic alone is insufficient in most cases and drainage is required. Delayed treatment and inadequate drainage carry a high risk of mortality and sequelae due to damage to the underlying joint. We present 2 cases of primary inguinal abscess in twin infants who presented at 1 month and 2 months of age correspondingly, in whom early diagnosis and management lead to quick recovery.
