BackgroundForeign body aspiration is one of the leading causes of childhood morbidity and mortality among older infants and toddler age groups. Missed and delayed diagnosis of foreign body aspiration can lead to increased incidence of complications. Early diagnosis can prevent life-threatening complications and morbidity. In this study, we aimed to evaluate the clinical and radiological details, types, localization of foreign bodies, complications, and outcomes in pediatric patients who presented to our hospital with foreign body aspiration.
MethodologyWe conducted a retrospective analysis of hospital case records of children aged one month to 14 years who were admitted to the Department of Pediatrics between June 2018 and May 2020, with clinical suspicion of foreign body aspiration.
ResultsA total of 22 children with a diagnosis of airway foreign body were included. The mean age of presentation was three years (SD: ±2.22), with a boy-to-girl ratio of 3.4:1. Cough (81.8%) and tachypnea (72.7%) were the most common clinical symptoms. The median duration between symptom onset and diagnosis was three (interquartile range: 6) days. Unilateral reduced breath sound (81.8%) was the most common clinical examination finding. The common site of impaction was the right main bronchus in 59.1% of cases. The foreign bodies retrieved during bronchoscopy were organic substances in 63.6% of cases, with peanuts being the most common (31.8%). Chest radiographs were normal in 36.3% of cases, and common abnormalities included hyperinflation, collapse, consolidation, and mediastinal shift. Mechanical ventilation was required in 54.5% of cases. The mean duration of hospitalization was five (SD: ±2.84) days. Complications such as pneumothorax were seen in one (4.5%) case. Mortality was seen in 4.54% of cases during the bronchoscopic procedures.
ConclusionsForeign body aspiration was common in young male children, with cough being the common symptom. Normal X-rays of the chest were seen in one-third of cases. The common site of impaction was the right main bronchus, and organic substances such as peanuts were common foreign bodies retrieved. Strong clinical suspicion of foreign body aspiration should be kept in cases with acute onset of cough in young children. Prompt medical attention is needed to reduce the morbidity and mortality associated with foreign body aspiration.
Objective: To assess the formula feeding practices in Bhubaneswar city and the parameters influencing it. Materials and Methods: A cross-sectional study was conducted from January 1, 2014, to December 31, 2014, in OPD and well-baby clinic of Hi-Tech Medical College, Bhubaneswar and pharmacies in the different Bhubaneswar Municipal Corporation units of this city. 250 mothers having infants and young children were interviewed individually as per pre-designed questionnaire. A separate set of questionnaires for 33 pharmacies were interviewed separately. The data were analyzed by SPSS 16.0 software. Results: About 38% of the mothers were preparing formula with proper dilution. 47% of mothers were adhering to a single brand. Formula sellingconstitutes 15-20% of the average total monthly income of pharmacies. 83% of the formulas were sold without prescription. Pharmacist (30%) and media (20%) were the major influencing factors. Conclusion: Mothers should be clearly explained about the health hazards of formula feeding during antenatal visits and subsequent immunization visits. Infant milk substitute act should include pharmacists, druggists, and chemists.
Effect of drugs often interplays with the diagnosis and treatment of patients presenting to Emergency and critical care services of the hospital. Urine drug screen (UDS) is a qualitative toxicological screening tool which is simple, fast and has a relatively low cost providing a clue in acute care settings. The present study aims to determine the prevalence of drugs of abuse determined by UDS in suspected patients in a hospital set up and compare it to national prevalence study. It is a retrospective study conducted in the Toxicology lab in KIMS Medical College and Hospital, in Odisha over a period of 30 months where urine samples received from the hospital were included. Commonly abused substances were detected by urine drug screen by the help of a commercially available lateral flow assay kit. We received 232 samples in this study period of which 108 (46.5%) tested positive by urine drug screen.The highest positivity among the samples was noted in the age group of 21-30 (33.3%) closely followed by 11-20 (30.6%) with a male preponderance.(32.3%) cases where single drug was found in urine drug screen while 33 (14.2%) of cases had multiple drugs detected in the UDS. Most common substance of abuse was benzodiazepines in 43.2% cases. THC (29.05%) and OPI (14.2%) were the other commonly abused drugs. In poly drug abusers, commonest drug combination consisted of OPI and THC with or without BZO in 36.36 % (12 /33) cases. Thus, in spite of having many limitation of urine drug screen will help in the acute patient care set up particularly for diagnosis of poly drug abuse.
