PD is a viable treatment option in older adults with stage 5 chronic kidney disease. Age alone should not preclude dialysis. Comprehensive geriatric assessment can prognosticate and facilitate shared decision making to commence dialysis in older adults.
Progressive supranuclear palsy (PSP) is a common type of atypical parkinsonism. To the best of our knowledge, there has been no study of its natural clinical course among Chinese patients. Methods: This retrospective study included 21 patients with PSP who had radiological evidence of midbrain atrophy (confirmed by magnetic resonance imaging) from the geriatrics clinics of Queen Mary Hospital and Tuen Mun Hospital. Clinical information was retrieved from clinical records, including age at onset, age at presentation, age at death, duration of symptoms, level of education, sex, presenting scores on Cantonese version of Mini-Mental State Examination, clinical symptoms, and history of levodopa or dopamine agonist intake and response. Clinical symptoms were clustered into the following categories and the dates of development of these symptoms were determined: motor symptoms, bulbar symptoms, cognitive symptoms, and others. Results: Motor symptoms developed early in the clinical course of disease. Cox proportional hazards modelling showed that the number of episodes of pneumonia, time to vertical gaze palsy, and presence of pneumonia were predictive of mortality. Coronavirus disease 2019 (COVID-19) disproportionately affects older people in Hong Kong. In Hong Kong, as of 17 July 2020, of the 10 recorded mortalities, seven involved patients aged ≥70 years. Social distancing as a strategy to limit the spread of COVID-19 has restricted the access to health and community services and has induced social isolation for older people. Hospital practice became less elderly friendly when infection control took priority over humanistic considerations. In 2004, The Hong Kong Geriatrics Society published a position statement on management of older patients with severe acute respiratory syndrome. Those guidelines were also followed during the current COVID-19 pandemic. Herein, we report our experience of caring for older people in Hong Kong during the COVID-19 pandemic, including the effect on older people of restricted access to health and community services, infection control measures in residential care homes for the elderly, treatment of older patients after admission to hospitals, end-of-life care, and the emergence of telecare.
A B S T R A C TFollowing a survey on the clinical practice of geriatricians in the management of older people with diabetes and a study of hypoglycaemia in diabetic patients, a round-table discussion with geriatricians and endocrinologists was held in January 2015. Consensus was reached for six domains specifically related to older diabetic people: (1) the considerations when setting an individualised diabetic management; (2) inclusion of geriatric syndrome screening in assessment; (3) glycaemic and blood pressure targets; (4) pharmacotherapy; (5) restrictive diabetic diet; and (6) management goals for nursing home residents.
