Introduction: An impacted tooth is a permanent tooth that has a completely developed root but which has failed to erupt. Many etiologies can lead to impaction, including a mechanical obstacle, such as benign tumor. Observation: An 11-year-old girl was referred for the avulsion of the left mandibular permanent canine, which was impacted. A radiolucent lesion with ground glass opacity was found on the eruption pathway of the tooth. Anterior radiographs showed the lesion that seemed to be the cause of the impacted canine. Anatomopathological examination revealed the lesion to be a juvenile trabecular ossifying fibroma. Comments: Ossifying fibromas are rare jaw tumors. They can be of various types: ossifying fibroma, psammomatoid, or trabecular juvenile ossifying fibroma. This is the first case that clearly shows the association between ossifying fibroma and tooth impaction. A literature review of ossifying fibromas and the management of fibro-osseous benign lesions of the jaws was performed. Conclusion: Although benign, juvenile trabecular ossifying fibroma can be locally aggressive and has a high rate of recurrence. It is very important to establish a precise diagnosis of a fibro-osseous benign lesion of the jaws to plan optimal management.
Introduction : Le succès esthétique et fonctionnel d'une chirurgie d'avancée mandibulaire dépend souvent de la qualité de la préparation orthodontique qui l'a précédée. Afin d'être optimale, cette dernière doit notamment chercher à reproduire un décalage dentaire équivalent au décalage squelettique de classe II pour permettre un geste opératoire d'une amplitude suffisante. Matériel et méthode : Le but de cet article est de présenter, grâce à un cas clinique, un nouveau dispositif d'ancrage postérieur mandibulaire, baptisé Abalakov, utilisé dans le cadre de ces préparations. Résultats : Les modalités de pose et d'utilisation de ce simple fil d'ostéosynthèse, placé dans la partie antérieure de la branche montante mandibulaire sont exposées pas à pas, jusqu'au résultat final. Discussion : Les avantages et l'efficacité de la technique sont ensuite discutés et comparés à d'autres alternatives de traitement. Conclusion : De là, il est possible de conclure que ce nouveau dispositif est un moyen simple, confortable, peu onéreux et surtout reproductible et très efficace de distalisation en masse de l'arcade mandibulaire, avec un redressement net des incisives. ABSTRACT -Presurgical orthodontic preparation of skeletal Class II by means of a bone anchorage with an osteosynthesis wire: a clinical case. Introduction:The aesthetic and functional success of mandibular advancement surgery often depends on the quality of the orthodontic preparation that preceded it. In order to be optimal, this preparation must try to reproduce a class II malocclusion equivalent to the sagittal skeletal discrepancy, to allow a surgical procedure of sufficient amplitude. Material and Method: The aim of this article is to present, thanks to a clinical case, a new mandibular posterior anchorage device, called Abalakov, used in the context of these preparations. Results: The modalities of placement and use of this simple osteosynthesis wire, placed in the anterior part of the ramus, are exposed step by step, until the final result is obtained. Discussion: The advantages and effectiveness of the technique are then discussed and compared with other treatment alternatives. Conclusion: It can be concluded that this new device is a simple, comfortable, inexpensive and above all reproducible and highly effective means of distalization of the mandibular arch, with clear uprighting of the incisors.
