Labio-palatin clefts are the most common type of craniofacial malformation encountered in practice and among the most common congenital abnormalities; it represent morphological and functional defects in newborns caused by genetically changes or environmental factors; those of the cervico-facial region have an increased incidence due to local embryological complexity and multifactorial etiology. The anomaly, with its variants, results from an inherent defect between the internal and external nasal buds, both from the frontal and upper maxillary bud. Labio-palatine clefts are characterized by a tissue deficiency as well as an abnormal disposition of the present tissues. It is believed that the labial clefts are consecutive to the lack of apoptosis of the ectoderm which envelops the internal maxillary and nasal buds, thus constituting a barrier to the fusion process of buds. Depending on the location and the extentension of the cleft, the labial cleft occurs, or the more frequent, the labial-alveolar cleft. The study group consisted of 22 patients with labio-palatine clefts, of which 16 boys (72.72%) and 6 girls (27.27%), who presented in the clinic for treatment. When the causative factor acts earlier, the clfet develops posteriorly, since mesodermization begins from the posterior to the anterior. The more disturbing factor acts, the greater the cleft. The increase in the incidence of labial-palatine cleft is believed to be mainly due to the increase in the number of women of older gestational age in women; women over 35 years of age have a double risk of having children with labio-palatine clefts, the risk being three times higher for mothers over the 39 years.
Anesthesia and the degree of control over the perception of pain depends on the personality of the individual, the socio-economic conditions, potential previous painful experiences and, last but not least, on fatigue and fear of the dentist. The perception of pain in patients is closely connected to their mental state. Pain is defined as a sensation of discomfort, with wide variations, both in quality and intensity, for different people in seemingly identical conditions; an unpleasant sensitive and emotional phenomena connected to the threat of a wound or caused in the tissues or described in the terms of this disease. The essential element of any type of anesthesia is analgesia, an effect which in some cases cannot be achived, due to the patient�s particularities or the physician�s lack of experience in anesthesia. Locoregional anesthesia (LRA) represents the blocking of the nociceptive sensitive and sympathetic autonomic afferents as well as that of motor efferents at the level of peripheral nerves� axons, by means of local anesthetic. To achieve the set purpose, we carried out a study on a representative human sample comprised of 10.123 patients treated in the Oral and Maxillofacial Surgery Clinic (Ambulatory) from the County Clinic Emergency Hospital St. Spiridon Iasi, between 01.01.2015-31.12.2016. The reason for the exclusion of certain categories of patients in the reseach was: the patients with a special conditions background require individual pre-anesthesia schemes, personalised for the nature of the pre-existing general condition, which must be further approved by the attending specialist physician : cardiologist, internist, diabetologist; children under 18 years old, with a high degree of anxiety; a high precentage of elderly patients, over 60 years old, possess a combination of general issues, thus requiring a special approach. The thoroughness lying at the core of the anesthetic practice, most especially the safegurading of a technical accuracy in the performance of anesthesia [12,], instead of improvisations, the lack of anatomical and stomatological training in general and the resulting inefficiency as such, is the underlying in-depth structuring element of this paper.
Colorectal cancer is a malignant disease with high morbidity and mortality. Ki-67 is a marker of cell proliferation associated with a low survival in colorectal cancer patients. Its expression is increased in patients with Dukes C or D colorectal cancer versus those in stage A or B. The purpose of the study is to evaluate the immunohistochemical expression of the Ki-67 marker in colorectal adenocarcinomas. The study was performed on a group of 28 patients diagnosed with rectal cancer. We made the histopathological and immunohistochemical evaluation of patients in the study group. The evaluation was performed on the sections stained with hematoxylin-eosin, and for mucinous forms, on sections stained with alcian blue. The topography of the Ki-67 reaction is strictly nuclear, evident in areas with intense proliferative activity. In cases with a strong positive response to Ki-67, the prognosis is worse, with invasion of the perirectal tissues and metastases. The lack of Ki-67 expression in nearby neoplastic tissues suggests that cancerous tissue proliferates in a pathway that it is not correlated with adjacent tissues. The Ki-67 could be useful in the prognosis of patients with colorectal cancer.
Anti-inflammatory-analgesic medication is known to have a wide spread, its indications going beyond the area of rheumatology, aimed at various fields, cardiology, nephrology, hematology,neurology, etc. Many years of aspirin has constituted the health expectation of millions of patients. Most nonsteroidal anti-inflammatory analgesics (ASNS) are acidic compounds derived mainly from carboxylic acids and enolic acids. The non-acidic compounds are numerically reduced and relatively unrelated. The main effects of non-steroidal anti-inflammatory analgesics arise following antipyretic action, analgesic action and anti-immflamatory in varying proportions to each structural group. Each drug has the specificity of single actions, the global way of explaining the clinical effects remains little known. Anti-inflammatory (anti-termic) in acute rheumatism or other inflammatory joint disorders, anti-platelet antiaggregant, aspirin prevents aggregation of blood platelets (which have a role in stopping bleeding).This is why it is used to prevent thrombosis (clotting of blood in the arteries or veins) with an impOliant role in preventing myocardial infarction. The study includes 126 patients who often used aspirin. Interaction of aspirin with other drugs mainly occurs in the plasma albumin, platelets, liver, kidney and gastrointestinal tract. Considered a common drug, often used by patients without the physician�s indication, some of them under maintenance medication (corticosteroid, anticoagulants, antiplatelet, antidiabetics, cytostatics), aspirin may cause important complications.
Spinal metastases from gastrointestinal (GI) cancer are rare and as a result there are only case reports or small series in the literature. The aim of our work was to identify the demographic aspects, the location, and the histopathological aspects of spinal metastases from GI cancers diagnosed and treated in a reference Hospital in Romania over a period of nine years, and comparing the data obtained with those from the recent literature. This is a retrospective case series study on spinal metastases from GI cancers, developed in patients older than 18 years that were surgically treated between January 2013 and December 2021 within three Neurosurgery Clinics from Prof. Dr. Nicolae Oblu Emergency Clinical Hospital, Iaşi, Romania, which is a tertiary Hospital in Romania regarding the surgical treatment of spinal metastases. We included in our study the patient’s demographic data (age and gender), clinical data (location of spinal metastases), radiological investigations and pathological features of the lesions. Regarding the immunohistochemical stainings, the following antibodies were used: anti-cytokeratin (CK)7, anti-CK20, anti-CK19, anti-caudal-type homeobox 2 (CDX2), anti-human epidermal growth factor receptor 2 (HER2), and anti-Ki67. Our series included 40 adult patients (≥18 years old) with a male:female ratio of 3:1, in favor of male patients. The mean age of all patients was 66.42 years. The primary sites of spinal metastases from GI cancers were from all segments of the GI system: the most frequent, however, was from the colorectal level (40%) and the least from the oral cavity level (2.5%). The most common site of spinal metastases from GI cancer was predominantly lumbar region (47.5%), and the most frequent histological type was adenocarcinoma (57.5%), followed by hepatocellular carcinoma (27.5%), pancreatic ductal carcinoma (5%) and squamous cell carcinoma (2.5%). Our results have important clinical implications because they suggest that there are certain subsets of patients with certain types of GI cancers that cause metastases in certain regions of the spine.
