Purpose: To evaluate the short-term visual outcomes of patients fitted with trifocal diffractive intraocular lens (IOL) who had retinal surgery (either received previously to cataract surgery or simultaneously with cataract surgery). Methods: A retrospective study of 22 eyes received trifocal diffractive IOL implantation. All cases had retinal surgery before or performed simultaneously (combined retinal surgery) with IOL implantation. All eyes received same hydrophobic trifocal diffractive IOL (Acrysof IQ PanOptix; Alcon Laboratories, Inc., Fort Worth, TX, USA). The uncorrected distance visual acuity at 4 m, uncorrected intermediate visual acuity at 60 cm, and uncorrected near visual acuity at 33 cm were evaluated at postoperative 1 week, 1 month, and 3 months. Results: Of the 22 eyes, 13 cases underwent cataract surgery after vitrectomy and 9 cases underwent simultaneous vitrectomy and cataract surgery. The most common cause of vitrectomy was the epiretinal membrane with 13 eyes. There were no significant vision-threatening complications, but one case showed early posterior capsular opacity at 3 months and vision restored immediately after YAG-laser capsulotomy. All post-operative visual acuity values showed statistically significant improvement compared to pre-operative values (p < 0.001). Conclusions: Trifocal IOL is a viable option in cataract patients with a history of retinal surgery or those who require combined retinal and cataract surgery.
