Purpose-The purpose of this paper is to present a structured way for the definitions of the Process Failure Mode and Effect Analysis (FMEA) attributes, such as potential failure mode, potential cause and potential effect, in order to make it simpler to define the controls and scores. Design/methodology/approach-This study performs, through a case study in incoming inspection of raw material, the comparison of a conventional application of the Process FMEA with a proposal based on the concepts of process approach defined by ISO 9001. Findings-Even written in a form similar to a script, the application of Process FMEA is a very complex activity and, like most quality tools, before being applied, FMEA should be clearly understood by the team. One way to facilitate this understanding is considering the sequence of events in the failures analysis to understand their causes and effects, just as are the sequences of inputs and outputs in the definition of the process approach addressed in ISO 9001. Originality/value-This paper shows a simple way to better structure Process FMEA, facilitating meetings with multidisciplinary teams.
<p>The Failure Mode and Effect Analysis (FMEA) has been developed by several researchers who have proposed distinct reference tables to score the Severity, Occurrence and Detection of a failure. This paper aims to evaluate, using AHP, several proposals for the application of Process FMEA, as well as to obtain and offer recommendations for its application. Five reference tables for Severity, six tables for Occurrence, and six tables for Detection are presented. These reference tables are evaluated for their application in the Brazilian automotive industry by using critical analysis and proper judgments. The scientific contribution of this paper is to provide a way to select a reference table, available in the FMEA literature, for the application of the FMEA process.</p><p>http://dx.doi.org/10.13033/ijahp.v2i1.69</p>
Este trabalho apresenta uma investigação sobre o emprego de FMEA (Failure Mode and Effect Analysis) de Processo com a exposição de irregularidades na sua utilização. O método AHP (Analytic Hierarchy Process) e os Conjuntos Fuzzy são aplicados no estudo das práticas atuais de utilização de FMEA. O AHP é aplicado para a priorização das irregularidades quanto à gravidade de sua ocorrência. Os Conjuntos Fuzzy são aplicados para avaliação do desempenho da utilização de FMEA em algumas empresas do ramo automotivo. Como resultado, tem-se a aceitação de oito e a não aceitação de três dos onze formulários de FMEA averiguados.
In order to create proper conditions to make projects successful, a growing number of organizations are taking up the practice of assessing Project Management Maturity (PMM). PMM can be verified through a variety of models. This paper reports the results of a survey, which used the Kerzner Maturity model, of 101 Brazilian companies. The novel and scientific contribution of this paper is the use of a Fuzzy Expert System as a tool to deal with the model's subjectivity and to check the consistency of survey's responses. The methodological approaches of previous Project Management Maturity studies included only trivial statistical analysis and usually showed less accuracy in the final results than that of the current research. The present work innovates by introducing the use of intelligent techniques, such as Fuzzy Sets and Expert Systems, in the Project Management Maturity analysis.
