Objectives To prospectively assess self-perceived chewing function (CF) and oral health-related quality of life (OHRQoL) in geriatric patients after receiving three different treatment modalities in the mandible: removable partial denture (CD-RPD), complete denture (CDs), or complete overdenture supported by mini dental implants (CD-MDI). At baseline, all patients had mobile anterior teeth (1 mm or >) and missing posterior teeth in the mandible. Patients were completely edentulous in the maxilla. After treatment, patients were recalled at the 3-month and the 2-year post-treatment period. Materials and Methods A total of 176 patients participated (CD group, n=68; CD-RPD group, n=58; CD-MDI group, n=50). Self-reported CF was assessed using the Chewing Function questionnaire (CFQ), The OHRQoL was evaluated using the OHIP14 questionnaire, which the patients completed 1. before treatment, 2. three months after treatment, and 3. at the 2-year post-treatment stage. Results The OHRQoL and the self-perceived CF significantly improved in all groups after treatment (p<0.01). The highest improvement of a CF was recorded in the CD-MDI group. The OHRQoL was significantly higher in the CD-MDI group in comparison to the CDs group after treatment (p<0.01). At the 2-year post-treatment stage, self-perceived CF significantly further improved in the CD-MDI group, while it worsened in the CD and the CD-RPD groups (p<0.01). The same pattern was recorded for the OHIP14 summary scores. The highest amount of denture repairs and adjustments was recorded in the CD-RPD group, although maintenance was also demanding in the CD-MDI group. Conclusion Within the limitations of this study, rehabilitation with mandibular MDI retained overdenture can be considered as preferred treatment with the constant improvement of OHRQoL and a chewing function in comparison to mandibular CD or mandibular RPD option in patients with mobile anterior mandibular teeth.
The aim of this study was to analyse short tandem repeat (STR) variation using the data on nine loci (D3S1358, vWA, FGA, THO1, TPOX, CSF1PO, D5S818, D13S317, D7S820) in the populations from eight villages on the island of Korcula, Croatia, in order to analyse its genetic and population structure. The analysis of STR data in this study indicated an appreciable degree of genetic homogeneity among the studied village populations on the island, even though a so-called 'east-west dichotomy' and differentiation between the inhabitants of the most recent settlement and the remaining ones was indicated with respect to the loci CSF1PO and TPOX, respectively. The validity of STR markers in assessing genetic structure of small populations and especially in determining the relationships among geographically closely related but reproductively isolated groups remains to be further evaluated, especially in terms of a larger number of studied loci in order to possibly find specific markers useful for resolving genetic patterns of variability at regional levels.
Zusammenfassung Fragestellung: Die radikale Hysterektomie nach WertheimMeigs wird als Standard der operativen Behandlung des Zervixkarzinoms im Stadium FIGO I b sowie FIGO II a betrachtet. Die erweiterte Radikaloperation schließt die Hysterektomie unter Einschluss einer Scheidenmanschette (Sicherheitsabstand über 2 cm), die Mitnahme beider Parametrien sowie der Ligamenta sacrouterina bis zur Beckenwand hin ein. Alternativ zur Radikaloperation besteht die Möglichkeit, in den genannten Stadien die Patientin einer Hochvolttherapie mit dem Linearbeschleuniger zuzuführen. Da hiermit eine neue strahlentherapeutische Option für die Behandlung des Zervixkarzinoms besteht, ist es um so wichtiger, die Nebenwirkungen der genannten Methoden miteinander zu vergleichen. Im Folgenden wollen wir auf operative Komplikationen der Wertheim-Meigs-Operation eingehen. Patientinnen und Methode: Zwischen 1999 und 2003 wurden in der Klinik für Gynäkologie und Geburtshilfe der J.-J.-Strossmeyer-Universität in Osijek, Kroatien, 46 Patientinnen mit einem Zervixkarzinom primär nach Wertheim-Meigs operiert. Dabei befanden sich 3 Patientinnen im Stadium I a 2, 34 Patientinnen im Stadium I b sowie 9 Patientinnen im Stadium II a. Die Patientinnen waren zwischen 26 und 76 Jahre alt. Postoperativ wurde bei 17 Patientinnen eine adjuvante Strahlentherapie durchgeführt. Anhand der 46 Krankenakten wurde eine retrospektive Abstract Background:The Wertheim-Meigs procedure (radical hysterectomy) is regarded as the standard surgical procedure for the treatment of cervical cancer in stages FIGO I b and FIGO II a. This extensive operation includes the resection of a vaginal margin (> 2 cm) and resection of parametric and sacrouterine ligaments. Alternatively, patients with tumors in these stages could be treated by radiation. Before chosing one of the treatment methods, it is important to consider the unwanted side effects of both options. We will focus here on surgical complications during and following the Wertheim-Meigs procedure. Patients and Methods: 46 cervical cancer patients received surgical treatment by the Wertheim-Meigs procedure at the J. J. Strossmeyer University, Osijek, Croatia, Department of Obstetrics and Gynecology. 3 patients had stage I a 2 disease, 34 patients had stage I b and 9 patients had stage II a cervical cancer. Patients were between the ages of 26 and 76 years. In 17 patients an adjuvant radiotherapy was performed. Patient charts of all 46 operated patients were reviewed and analysed retrospectively. Complication rates were compared and discussed after reviewing the recent literature. Results: In 22 patients (47.8 %) a medium strong bleeding (blood loss of 1000 -2000 ml) occurred intraoperatively. A severe blood loss (> 2000 ml) occurred in one patient (2.2 %).
