Objectivesthe purpose of this study was to compare the postoperative radiological and clinical outcomes with minimally invasive percutaneous osteosynthesis using three implants: volar locking plate, intramedullary nail system and nonbridging external fixator for distal radius fractures.Methodsforty-eight patients (A group, 16; B group 16; C group 16) underwent minimally invasive percutaneous osteosynthesis of reductible and unstable displaced (Type IIB by Rayhack Classification) distal radius fractures. In B group intramedullary nail system was used, in A group the patients were treated with volar locking plate and in C group the patients were treated by nonbridging external fixator from January 2011 to December 2012. The mean follow-up period was 12 months. Radiologic parameters, range of motion, grip strength, and disability of the arm, shoulder, and hand score were evaluated at each examination (3rd and 6th week, and 12th months). The visual analog scale of wrist pain and complications were assessed at the final follow-up.Resultsthe groups did not differ significantly in radiological outcomes after 12 months, but the clinical results, VAS scale and dash score in group A (volar locking plate) and B (nail intramedullary) were statistically significantly better than that of C group (nonbridging external fixator). One patient underwent an osteosynthesis with nail intramedullary and another with external fixator (C group) developed persistent pain near the site of the superficial radial nerve because of the distal's screw and pins, respectively.Conclusionin clinical parameters, significant differences in outcomes were found between groups A and B after six weeks versus C group.
ObjectiveTo present the clinical and functional results, including measurement of quality of life, of patients undergoing trapeziometacarpal arthroplasty.MethodThis was a prospective evaluation on 45 patients (53 thumbs) with a diagnosis of idiopathic rhizarthrosis who underwent resection arthroplasty and interposition of an uncemented Ascension® implant, made of pyrocarbon. The clinical and functional results were analyzed through radiography, range of motion (ROM) in degrees (°), visual analog scale (VAS) for pain and the disability of arm, shoulder and hand (DASH) questionnaire for quality of life. In the group analyzed, 38 were women and seven were men, and their mean age was 63.17 years (range: 50–78). Eight patients were treated bilaterally.ResultsAfter 42.08 months of follow-up (range: 8–73), the subjective pain evaluation (VAS) score was 1.37 (range: 1–4). The complete ROM of the thumb increased to 95.75% (range: 75–100%) in relation to the contralateral side. The mean DASH questionnaire score was 9.98 (range: 1–18). The complication rate (negative events) was 11.32%. Five patients presented dislocation of the thumb prosthesis. All of them were reoperated by means of dorsal capsuloplasty using a portion of the retinaculum of the extensors as a graft, and good clinical evolution was achieved in these cases. One patient presented fracturing of the metacarpal and was treated by means of osteosynthesis using Kirschner wires.ConclusionThis method is effective for treating rhizarthrosis, according to the measurements made on the clinical and functional results, even after taking the complication rate into consideration. Moreover, it provides an improvement of quality of life for these patients.
Introdução: A poluição atmosférica urbana produz efeitos adversos sobre a saúde dos grupos mais suscetíveis. Objetivo: Avaliar os efeitos da exposição à poluição do ar sobre as internações hospitalares por insuficiência cardíaca congestiva de adultos e idosos residentes na cidade de Santo André. Métodos: Foram obtidas as internações por insuficiência cardíaca congestiva (ICC) de adultos e idosos (35 anos ou mais) em todos os hospitais conveniados ao Sistema Unico de Saúde, na cidade de Santo André, para o período entre 2000 a 2007. Os níveis de PM10, temperatura e umidade foram fornecidos pela CETESB. Foi utilizado modelo linear generalizado de regressão de Poisson e um modelo de defasagem de até 20 dias com distribuição polinomial de sexto grau para estimar os efeitos do poluente no desfecho, controlando-se para sazonalidades de longa e curta duração, feriados e fatores meteorológicos. Resultados: A média do PM10 durante o período do estudo esteve abaixo do padrão de qualidade do ar adotado pela CETESB, que é de 50 μg/m3, e o valor máximo diário observado também não ultrapassou o limite de 150 μg/m3. Para um aumento de 24,6 μg/m3 de PM10, foi observado um aumento de 3,0% (IC95%: 0,3-5,7) nas internações por insuficiência cardíaca congestiva no mesmo dia da exposição. O efeito do poluente sobre o desfecho apresentou comportamento bifásico, constatando-se novo aumento nas internações 10 a 12 dias após a exposição. Conclusões: Aumentos na concentração de PM10 na cidade de Santo André estão associados a aumentos nas internações por ICC entre os adultos e idosos.
rESUMo introdução: A utilização de técnicas minimamente invasivas para o tratamento de fraturas do rádio distal teve um aumento significativo nos últimos três anos. Isso é explicado devido ao aumento na incidência de fraturas em pacientes economicamente ativos e à necessidade destes retornarem precocemente ao trabalho. objetivo: Analisar os resultados clínico-funcionais dos pacientes com diagnóstico de fratura com desvio redutível e instável da extremidade distal do rádio submetidos ao tratamento cirúrgico pela técnica de osteossíntese percutânea e minimamente invasiva utilizando haste intramedular bloqueada. Métodos: Foram avaliados 53 pacientes submetidos ao tratamento cirúrgico da fratura da extremidade distal do rádio, redutível e instável, classificadas segundo Rayhack como Tipo IIB, pelo método minimamente invasivo, com haste intramedular bloqueada no período entre janeiro de 2011 e dezembro de 2012. O tempo de seguimento foi de seis a 24 meses, avaliando periodicamente parâmetros radiográficos, dor, medida do arco de movimento, força de preensão palmar e qualidade de vida através do questionário DASH. resultados: Através da análise dos valores obtidos observamos uma melhora estatisticamente significante em todos os parâmetros clínicos analisados. O grau de força de preensão palmar e a medida do arco de movimento apresentaram melhora importante desde a terceira semana de pós-operatório, assim como a escala de dor e a avaliação do questionário DASH. Conclusão: A técnica minimamente invasiva é eficaz e segura, com melhora clínica e funcional e apresenta resultados superiores quando comparada a outros métodos descritos na literatura. ABStrACt introduction:The use of minimally invasive techniques for the treatment of the distal radius fractures has been increasing significantly over the past three years. This increase can be explained by the rising incidence of fractures in economically active patients and their need to early return to work activities. objective: To analyze the clinical and functional results of patients diagnosed with distal radius fracture with reducible and unstable deviation, who underwent surgical treatment by the minimally invasive percutaneous osteosynthesis technique using a locked intramedullary nail. Methods: We evaluated 53 patients who underwent surgical treatment of reducible and unstable distal radius fractures, classified according to Rayhack as type IIB, using the minimally invasive method with intramedullary locked nail in the period between January 2011 and December 2012.
