The research is aimed at finding ways to effectively integrate traditional career guidance approaches and the capabilities of digital platforms working in the field of youth policy management in the Sverdlovsk region. The ways of intersection of interests of universities and youth policy management bodies in the emerging unified Internet space are outlined. The analysis of digital transformation process in the field of career guidance is carried out. The possibilities of the main federal information platforms in the field of youth work and their effectiveness in terms of indicators are presented: the number of events, the coverage of participants and, what is important for us– the project activities of young people with a career-oriented component. The article provides examples of social projects that already have a digital footprint and a sufficiently large involvement of schoolchildren and students of the Sverdlovsk region. The educational component containing elements of career guidance, which can be further strengthened, is of particular interest. The ways of creating a territorial multilevel digital model of youth career guidance of the Sverdlovsk region are outlined.
Acute cerebrovascular accident is one of the major causes of death and disability of patients around the world. In 30-40% of cases, the cause of ischemic stroke is an extracranial lesion of the brachiocephalic vessels. Carotid endarterectomy is the main technique for preventing cerebrovascular accident in carotid artery stenosis. General anesthesia with endotracheal intubation has been the method of choice since the widespread introduction of this type of surgical intervention into clinical practice and in most centers remains so to this day. At the same time, many authors report, that the use of general anesthesia with endotracheal intubation has limitations in a number of patients, namely, in the presence of an embologenic plaque in the operated internal carotid artery, severe coronary artery disease, decompensated aortic and / or mitral stenosis, low ejection fraction, heart rhythm and conduction disorders, severe chronic diseases of the respiratory system, as well as patient refusal for general anesthesia.THE AIM of the study was to show the possibilities of using regional anesthesia for carotid endarterectomy in a high-risk patient.RESULTS The use of regional anesthesia allows surgeons to avoid instability or abrupt changes in hemodynamics in the intraoperative period. Indications for its use are also cases of critical contralateral stenosis of the internal carotid artery when it is necessary to carefully consider the advisability of installing an internal stent.CONCLUSION The use of regional anesthesia made it possible to successfully perform carotid endarterectomy without cardiovascular events, neurological deficit and postoperative complications in a patient with relative contraindications to general anesthesia.
Представлено клиническое наблюдение послеоперационного обезболивания пациентки после тотального цементного эндопротезирования правого тазобедренного сустава в раннем после операционном периоде. Предложена перикапсулярная блокада (Pericapsular nerve group block, PENG-block) тазобедренного сустава с целью адекватного обезболивания и снижения риска послеоперационных осложне ний. Данное клиническое наблюдение показывает пример эффективного обезболивания пациентки в раннем послеоперационном периоде после реконструктивнопластического восстановления правого тазобедренного сустава, выполненного по поводу перелома шейки бедренной кости. Выраженный болевой синдром, ограничение движений, вынужденное положение и объем опе ративного вмешательства являются факторами риска развития тромбоэмболических осложне ний. По данным литературы, частота развития тромбоэмболических осложнений после тоталь ного цементного эндопротезирования тазобедренного сустава варьирует в пределах от 9,3 до 20,7%. Наше наблюдение свидетельствует, что перикапсулярная блокада тазобедренного сустава является эффективным методом обезболивания в раннем послеоперационном периоде после тотального протезирования тазобедренного сустава.
This paper considers the implementation of the Strategy for the development of the agrobusiness of the Sverdlovsk region. The conclusion is confirmed concerning insufficient development of the business environment in the agrarian sector. Approaches and methods of transformation of the classical agrarian university into an entrepreneurial one on the example of the Ural State Agrarian University are discussed: knowledge generation, teaching and translation of knowledge into practice; reshaping of the university’s internal environment; modification of interactions with the external environment developing the university’s innovation environment. The results of the project and their significance are predicted: the creation of a modern system of educational, scientific and design-entrepreneurial environment. New educational products will be developed. A set of indicators and marks of compliance of the agrarian university to the entrepreneurial type will be introduced. The uniqueness of this project has been found. The thing is that the Ural State Agrarian University is not a classical or purely technical university and this expands its ability to enter various areas of the region’s life: economic, social and technological. The article concludes that the implementation of the project will meet the needs of the agriculture cluster of the Sverdlovsk region in the development of intellectual entrepreneurship and avoid primitive mechanisms of collection and independent implementation of their agricultural products by residents.
