Smoking related interstitial lung disease Inhalation of tobacco smoke is a risk factor for developing respiratory diseases as chronic obstructive pulmonary disease, lung cancer and many cardiovascular diseases. Recently, a new group of interstitial lung diseases (ILD) related to cigarette smoking (SR-ILD) have been described. This group includes pulmonary Langerhans cell histiocytosis, respiratory bronchiolitis, smoking-associated interstitial fibrosis, desquamative interstitial pneumonia. The diagnosis is usually difficult, and the use of some clinical clues, high-resolution computerized tomography, and histopathologic findings in lung biopsy could help to differentiate between the various entities. We present a report of clinical cases of patients with SR-ILD seen in our center, and a review of the literature of the above entities.
Paciente de 66 años, de origen rural, con antecedentes de tabaquismo de 20 paquetes-año. Debido a un cuadro de disnea progresiva y tos en el año 2002, se realiza estudio, diagnosti-cándose una enfermedad pulmonar difusa tipo UIP (neumonía intersticial usual), por biopsia quirúrgica, recibiendo varias veces tratamiento con corticoides en forma intermitente. A fines del año 2005 sufre deterioro progre-sivo de su capacidad funcional, y baja de peso, con compromiso del estado general. La espiro-metría resultó dentro de límites normales, la ca-pacidad de difusión (D LCO) severamente dismi-nuida (42%), con desaturación durante el ejerci-cio en la caminata de 6 minutos.
