В обзоре представлены данные литературы последних лет о патогенезе, диагностике, лечении амиодарон-индуцированного тиреотоксикоза (АИТ), который является частым осложнением терапии амиодароном (Ам). Описаны изменения секреции и метаболизма тиреоидныхгомонов под влиянием кратковременной и длительной терапии Ам. Развитие АИТ всегда приводит к ухудшению течения аритмии, усугублению недостаточности кровообращения, утяжелению состояния пациента. Выделяют АИТ 1 типа и АИТ 2 типа, а также смешанную форму. Описаны диагностические критерии АИТ 1 и 2 типа. Наиболее информативным тестом для дифференциальной диагностики АИТ 1, 2 типов и смешанных форм является сонография щитовидной железы с допплеровским исследованием кровотока и сканирование с 99mTc-sestaMIBI. Тактика лечения определяется типом АИТ, состоянием сердечно-сосудистой системы, риском повторных аритмий. Консервативное лечение зависит от типа АИТ и осуществляется тиреостатиками или глюкокортикоидами. Обсуждается возможность продолжения антиаритмической терапии Ам у пациентов, перенесших АИТ. При АИТ 1 типа и смешанной форме требуется отмена препарата, если это невозможно – радикальное лечение тиреотоксикоза (радиойодтерапия, тиреоидэктомия). АИТ 2 типа является самолимитирующимся процессом, при жизненно угрожающих аритмиях прием Ам может быть продолжен. Показана эффективность радиойодтерапии для радикального лечения тиреотоксикоза при АИТ 1 и 2 типа, несмотря на низкий захват радиойода. Однако этот вопрос требует дальнейшего изучения и обсуждения. Для быстрого восстановления эутиреоза у тяжелых больных применяют плазмаферез и тиреоидэктомию.

Aim to improve the dispensary follow-up of patients with type 2 diabetes (DM) by identifying clinical signs, available during examination in the outpatient clinic, that suggest the development of chronic kidney disease (CKD). Material and methods. A cross-sectional study included a population sample of patients with type 2 diabetes and focused on the clinical indices available during an outpatient consultation. 150 protocols of outpatient consultations were selected using the compliance criteria, including data on 72 clinical indices. The general sample was divided into groups in two ways: by the level of glomerular filtration rate (GFR) and by achieving the goals of glycemic control (calculated parameter delta HbA1c). Results. The following indices differed most significantly in the groups divided by stages of CKD: gender, age, duration of diabetes and insulin therapy, achievement of target values of glycemic control, blood pressure level, comorbid cardiovascular pathology, pulse on the popliteal artery. In patients with preserved GFR and the early stages of CKD, the fact of unsatisfactory glycemic control was associated with an increase in plasma creatinine and a decrease in GFR, higher blood pressure values. In the advanced stages of CKD, the impact of glycemic control on kidney function was practically absent.

Significance: analysis of the mortality structure of patients with diabetes mellitus makes it possible to obtain information not only about the disease outcomes, but also to assess dynamics in epidemiological indicators of complications of diabetes mellitus, especially their terminal stages, which is one of the key effectiveness indicators of medical and rehabilitation measures. The purpose of the study: to conduct a dynamic analysis of the level and structure of mortality of patients with diabetes mellitus in the Samara region for the period from 2000 to 2017. Material and methods: the object of the study was a depersonalized database of the state register of diabetes mellitus in the Russian Federation and the Samara region and the death registration form of patients with diabetes mellitus of the annual report on diseases of the endocrine system. The mortality rates were calculated, the structure of mortality of patients with diabetes mellitus was analyzed according to the ICD-10 disease classes individually for diabetes mellitus type 1 and 2. Results: trends in the dynamics in mortality of patients with diabetes mellitus in the Samara region generally correspond to the Russian ones. Significant differences in the trends in mortality rates in patients with type 1 diabetes mellitus have been identified: the mortality rate in the Samara region increases against the background of the decreased indicator in the Russian Federation, and different vectors of the dynamics in mortality from atherosclerotic cardiovascular diseases are registered. Conclusion: the main reason for changes in the long-term decreasing trend in mortality from diabetes mellitus in the Samara region in 2010-2017 was changes introduced to the procedure for registering causes of death. When entering data into the state register of diabetes mellitus, if the cause of death is indicated as "diabetes mellitus", it is mandatory to indicate the direct cause of death. A limited list of basic mortality indicators of patients with diabetes mellitus is proposed, which can serve as indicators for the dynamic assessment of the epidemiological situation with complications of diabetes mellitus, as well as objective criteria for effectiveness of the ongoing medical and rehabilitation measures.
