This article provides a systematic review of studies on the composition of leech saliva secretions, the method of hirudotherapy, and the results of hirudotherapy in complex treatment. The following databases were used to search for the study: Scopus, Web of Science, MedLine, and Global Health. When using known methods of treatment, there is often no stability of results, this is explained by the search for new drugs and methods for treating periodontitis. It is known that the secret of the salivary glands of the medical leech contains more than 100 biologically active substances. The therapeutic effect of hirudotherapy is associated with the mechanical discharge of blood flow and the action of a complex salivary gland secretion of a medical leech. The molecules that exist in leech saliva and are the most studied to date include active substances such as hyaluronidase, hirudin, Kalin, destabilase, bdellins, histamine-like substances, hirustazine, factor XA inhibitor, and collagenase. It is scientifically proven that hirudotherapy leads to a significant improvement in the clinical condition of periodontal tissues, is simple, accessible, and can be recommended for use in the practice of periodontists.
In the experimental study, the penetrating power of dental temporary filling materials Cresofen, Eugenol, Cresodent, Calasept was analyzed. The assessment was made by studying the diffusion of colored solutions of drugs. As a dye used jodoin. It was revealed tha t preparations for temporary filling had different penetrating ability. The greatest penetrating power in the tooth tissue from the studied antiseptic solutions has Cresophen, which penetrates to the entire depth of the tooth tissues after 3 days after application, the intermediate value was shown by Eugenol and Cresodent, which penetrated the cervical and apical regions after 7 days, and Calasept had minimal penetrating properties. These data should be taken into account when planning treatment activities.
This article reviews research data on the etiology, pathogenesis, and features of inflammatory periodontal diseases, the possibilities of using hirudotherapy to treat this group of diseases, and the mechanisms of action of this method. The literature search was conducted using the databases Scopus, Web of Science, MedLine, and Global Health. The mechanisms by which medical leeches influence the human body were analyzed. It is known that the progression of inflammatory periodontal diseases depends on risk factors such as age, smoking, gender, socio-economic factors, and some systemic diseases. The main features of chronic and acute periodontitis are periodontal pockets accompanying the loss of bone mass and attachment of the tooth to the alveolar process, which occur unevenly throughout the dentition. The decisive factor in inflammatory periodontal diseases is the presence of microorganisms. Hirudotherapy is a treatment method using the medical leech Hirudo medicinalis. In their secretion, leeches have more than 20 bioactive molecules with various effects. Their therapeutic mechanisms include six subtypes: a decrease in the content of cellular elements and neutrophils in mixed saliva, normalization of the microflora of the periodontal pocket, and an increase in the number of lactobacilli. The treatment method using leeches leads to a decrease in the depth of the periodontal pockets, normalization of the periodontal index, and tooth mobility. Hirudotherapy leads to a noticeable improvement in the clinical condition of periodontal tissues, is convenient to perform, affordable, and can be recommended for use in the practice of periodontists.
This article provides a systematic review of studies on a new biological treatment method called regenerative endodontic technique. Databases of Scopus, Web of Science, and MedLine were searched for relevant studies. Dental trauma most common occurs in children and the most common cause of the loss of viability of the pulp of permanent teeth with incomplete root development. Treatment of these teeth takes time and technically complex. Thus, a technique was proposed that could use the potential of apical papilla stem cells, leading to the refilling of the root canal with vital tissue through the regenerative endodontic technique. It is necessary to pay due attention to the use of regenerative endodontic techniques, especially in cases where the root development is incomplete. The use of mineral trioxide aggregate is unlikely to improve the condition of the tooth.
Chronic generalized periodontitis (CGP) is a disease associated with low susceptibility to the therapeutic protocols applied; practitioners tend to characterize it as a disease presenting certain difficulties. Therefore, the search for drugs and methods capable of increasing the efficacy of CGP therapy is an ongoing process. Additional problems, which have to do with either with antibiotic resistance or increased sensitivity to drugs, also occur quite often. This study aimed to assess the possibility of applying hirudotherapy in the context of conservative treatment of CGP. 50 patients with CGP without somatic pathology were examined and treated. The participants were divided into two groups (n = 25), all group members of about the same age. At the first stage, the treatment followed the accepted standard: professional oral hygiene procedures, antimicrobial and anti-inflammatory drugs, demonstration of proper personal oral hygiene routines. Then, first group went through a monthlong hirudotherapy course that consisted of 6 to 8 individual procedures. Second group was observed throughout this period with the aim to control the level of their compliance with the oral hygiene routines they were trained. Having analyzed the results, we found that hirudotherapy was more effective than what was prescribed to the second (control) group. The papillary marginal alveolar index (PMA), which reflects the severity of inflammation and gum bleeding, decreased significantly in the first group, where medicinal leeches were used: in the patients with severe CGP it went down by 6%, in those with moderately severe CGP the index decreased by 24% and the participants whose CGP was only light had the PMA go down by 2%. Thus, we have demonstrated the efficacy of hirudotherapy in the context of conservative CGP treatment, which allows recommending this method for inclusion into clinical practice.
