IntroductionDandy-Walker syndrome is a congenital brain malformation. The major presentations are mental disorder, cerebellar ataxia and symptoms related to hydrocephaly. It can be associated with psychiatric symptoms. Although, since this presentation is less prevalent, it has been insufficiently discussedCase presentationThe subject is a 17 year old girl suffering from mental retardation. Her signs started 3 months ago when she was considered as having decreased need to sleep and walking during the night mostly. Her appetite increased as well. she was found to be getting more talkative than past; especially she talked to herself more. She was admitted to hospital with the diagnosis of bipolar I disorder (the most recent episode: mania) in axis I and deep MR in axis II. In her brain MRI, enlargement of fourth ventricle and cisterna magna and atrophy cerebellar vermis were seen. EEG showed unspecific changes and there was no positive finding in her blood count, electrolytes, renal and liver function tests and blood sugar.Despite two weeks of treatment with risperidone 3 mg/day and biperidine 4 mg/day and partial control of some symptoms, irritability and anger remained consistently. So, valproic acid (VPA) 400 mg/ day was added to treatment and after one month the patient was discharged from hospital with complete remission of newlypresented symptoms.DiscussionThis report indicates the importance of taking psychiatric symptoms into consideration while evaluating organic brain disorders. With respect to treatment, the same treatment as primary mood disorders was indicated effective.
We describe the case of an 11-year-old boy with bipolar-I disorder comorbid with panic disorder who developed manic symptoms with a single dose of 10 mg of propranolol. Results and Conclusion Although depression is a better-known side effect of b-adrenergic antagonists, clinicians should take mania as a rare side effect into consideration as well.
Dermatitis artefacta or factitious dermatitis is a quite rare kind of psychiatric disorder which has been always a challenge for psychiatrists and dermatologists. The patient impulsively or as a result of primary or secondary gain makes deliberate scars with his hand or needle, pin, razor and … on his body. Based on kind of spread, shape, margin, and recognition of basic conflict, these scars can be differentiated from other dermatological ones. What makes this disorder a challenge is not its diagnosis but its treatment, and what is important in treatment understands its meaning by its patient. The researchers in this study introduce a married 45 years old woman, with Dermatic artefacta, whose scars are limited to her feet. In order to understand the meaning of this disorder, the following question is asked: Why the scars are just seen in her feet?We try to answer this question with the focus on childhood history, Iranian culture, worldwide common beliefs, literature, and idioms.
Fetishism is a kind of Paraphilia which is always observed in males. The beginning of this disorder is in youth .In Fetishism, the patient prefers to have sexual satisfaction with inanimate objects. The use of substance can cause some disorders in sexual functions. Amphetamines are addictive which their use has been raising recently, and from this group, one type is called Crystal. At first, this substance causes sexual reinforcement, but for long period of use, it causes sexual dysfunction. The author introduces a case, who got fetishism after use of crystal. The patient is a divorced man, and 41 years old, who started substance abuse several years ago, but since 7 months ago, he has used crystal, and now, just using crystal. Since starting the use of crystal, his sexual satisfaction is only with wearing women´s underwear, and no other way. Less use causes less desire. Before the use of crystal, there was no sign of paraphilia. A lot of symptoms which related to new substances are still new for psychiatrists who are treating patients in clinics, and this case is the first reported kind. Apart from introducing the patient, different issues about psychological symptoms of crystal are discussed.
