Objective: to describe risk management implementation in the safety of patients with mental disorders through action research. Method: an action research carried out with a multidisciplinary team from a reference hospital in mental health in Southeast Brazil. Results: three strategies considered as managerial technologies for the mentally ill patient were developed: risk management diagram for patient safety; patient safety protocols; and textual proposal of software for internal management of incident notifications. Final considerations: this action research allowed a collective discussion by the whole multidisciplinary team, enabling strategy drafting for risk management implementation and improvement of care quality in the safety of patients with mental disorders at the study site.

Objective: To describe the development of management technologies for risk management in the safety of patients with mental disorders. Method: A research study, conducted in 17 seminars with 13 members of the multiprofessional team from a reference hospital in mental health in a state in the southeastern region of Brazil. Results: Two management technologies were developed and validated: Risk management diagram and protocols, both focusing on the safety of patients with mental disorders. Conclusions and implications for practice: The safety protocol of the patient with a mental disorder and the diagram in a tree of risk management were two management technologies constructed in this study. Both can offer the nurse and multidisciplinary team a baseline to qualified care, grounded in patient safety principles.

A gestão de riscos de saúde é o processo que organiza, planeja, controla e dirige os recursos para minimizar ou controlar os riscos de saúde dentro da instituição. Os riscos nos serviços de diagnóstico por imagem são gerenciáveis e evitáveis, porém não há ferramenta específica para o gerenciamento de riscos e segurança do paciente. Desenvolver um software para gerenciamento de riscos de saúde no serviço de diagnóstico por imagem. Pesquisa aplicada de desenvolvimento tecnológico, realizada em três etapas: revisão integrativa da literatura, etapa Delphi online e construção do software. O software Quality Diagnosis é uma tecnologia que identifica, analisa, avalia, trata e monitora falhas de processo e eventos adversos nos serviços de diagnóstico por imagem com a execução de ações preventivas e corretivas identificadas durante as investigações de causa raiz. O software poderá contribuir para implementação da gestão da qualidade do Programa de Acreditação em Diagnóstico por Imagem, visando disseminar a cultura de segurança do paciente e garantir um atendimento de qualidade para todos os pacientes.
