The present report on 67 patients, in whom a ureterosigmoidostomy was performed on the ground of a benign indication, is to take a critical attitude towards ureterosigmoidostomy. In about 60 per cent of the cases satisfactory or good results can be achieved. Final comparisons with other methods of derivation cannot be made, since a sufficiently great number of late results is not yet available. In our opinion, it is doubtful if there will ever be an alternative. The operator's judgment and experience will always play an important role. Certainly the results can be improved by a lifelong close observation of the patients.
Innerhalb von 25 Jahren wurde in unserer Einrichtung bei 354 Patienten eine Harn-leiterdarmanastomose durchgeführt. 97 Patienten konnten nachuntersucht werden. Dabei wurde das Hauptaugenmerk auf Veränderungen des Mineralhaushaltes gelegt. Na+-, K+-, Ca++-, Mg++- und CI––-Bestimmungen im Serum wurden durchgeführt und die Ursachen ihrer Abweichung diskutiert. Ausserdem wurden Alkalireserve- und Harn-stoffuntersuchungen beim gleichen Patientenkreis vorgenommen. Die pathologischen Werte unserer Untersuchungsergebnisse haben wir prozentual aufgeschlüsselt und ihre Bedeutung für die Klinik aufgezeigt. Diesen Hinweisen folgte die kritische Einschätzung der Harnleiterdarmanastomose.
AFP was determined simultaneously in the sera and in the tumorous tissues of 10 patients with testicular tumour. Two seminomas proved to be negative with two methods. Out of the 8 non-seminomas 7 showed increased serum levels and AFP was detectable in the tumour tissues of 6 patients. Staining of AFP in the testicular tumour tissue with an indirect immunoperoxidase reaction has achieved clinical significance only in cases of metastases of unknown primary tumours or of extragonadal germinal cell tumours.
At the time of operation advanced carcinomas of the urinary bladder often have developed subclinical metastases. In 20 patients with metastases, inoperable carcinoma of the bladder responded to chemotherapy with a combination of adriamycin and 5-FU. 55% of the patients experienced either a complete or incomplete remission, which lasted long in some cases. In the future, suitable combinations of chemotherapeutics could participate in improving the prognosis of advanced carcinoma of the urinary bladder.
