The report concerns the techniques of cholecystectomy and choledochotomy employed in 7198 bile-duct operations comprising 9.5°/0 of all operations over a 121/2-year period. The transrectal longitudinal incision is preferred. Generally a bacteriological examination and cholangiography are carried out, but only in about 25°/o of cases is manometry performed. The results yielded by tele-cholangiography and documentary X-rays care complementary. The gallbladder bed is drained in every case. The preferred method of removing a choledochus stone is flushing. The choledochus is closed primarily in 61 °/o of cases. In 27 °/o a T-drain is installed. Only in 5 °/o of our cases was sphincterotomy performed. Total mortality was 3.8°/0. Zusammen/assunff. Nur in 5°/0 wird eine Sphincterotomie ausgeffihrt. --Die Gesamtletalit/it betr/~gt 3,8°/0.Ohne Zweifel ist es eine der Folgen unseres Wohlstands, daI~ wir alle seit Anfang der 60er Jahre mit Gallenwegs-und auch Pankreas-Chirurgie viel und immer noch zunehmend zu tun haben.
Between 1975 and 1983 38 patients with a fracture of the acetabulum were treated - 28 patients were operated upon, 10 patients were treated conservatively. 26 in 28 patients treated operatively and 8 in 10 patients treated conservatively could be checked up. 3 of the cases treated operatively could not be evaluated, as the operation had been realized less than two years before. Following an accurate indication for the operative treatment 14 in 23 patients proved an efficient osteosynthesis. In 5 in 8 cases treated conservatively good results were obtained. In aged patients and in certain types of fractures of the acetabulum, especially in comminuted fractures, conservative treatment is still justified.
Among 1146 patients with diseases of the pancreas observed over a 15-year period, there were 234 cases of malignant duodeno-pancreatic tumors. This report concerns the intraoperative diagnostic methods and the various palliative operations available. There was a ratio of 13 males to 10 females, and the average age was 63 years. In 24.3 percent of the patients attempts to establish the diagnosis intraoperatively by puncture or excision were unsuccessful. The cardinal symptom in 71 percent of these cases was icterus. Duodenopancreatectomy was possible only in 21 cases (= 8.9%). In 173 cases (= 75.2% of all patients) a palliative operation was performed.
The influence of the nail wall thickness on the stress distribution is shown for the AO-nail as well as the Küntscher-nail. The quality of the analytical solution is proved by demonstrating that the presence of the slot in the nail does not shift the bending stresses merely. The gap between nail and bone is shown to be responsible for misalignments in the fracture.
131 patients with malignant stenoses in the esophagus-cardia-region were palliatively treated with an endoprosthesis. The procedure in these patients of an average age of 65.5 years is described. The average in patient time is 22 days. The postoperative mortality is 22.9%. The complications specific for the operation method are dislocation of the prosthesis, perforation of the esophagus, bleeding and occlusion. 74.4% of the patients subjectively considered the operation successful.
