The objectives of this study were to determine the importance of diagnostic laparoscopy for the accurate diagnosis of endometriosis and to correlate the findings with infertility. Participants in this study included 336 women who were 18-45 years old, had no past medical history of abdominal operations, and complained of chronic symptoms of pelvic pain. In all these cases there were no pathological pelvic ultrasound findings. Also, nongynaecological diseases where excluded. Diagnostic laparoscopy was performed in all patients. In 191 women (56.8%) no pathology was found during the diagnostic laparoscopy, and 89 women (26.4%) actually reported improvement or even complete cure from their symptoms after the operation. In the majority of pathological cases the laparoscopy revealed various stages of endometriosis (n=101; 30%). Other gynecological causes which were diagnosed during the laparoscopy where pelvic adhesions due to inflammatory disease (n=37; 11%), ovarian cysts (n=5; 1.5%), and uterine fibroids (n=2; 0.5%). Diagnostic laparoscopy is the most accurate method for excluding the pathology related with chronic pelvic pain. Endometriosis seems to be responsible for the majority of pathological cases. Almost 60% of women have no pathology when examined with laparoscopy. A high percentage of symptoms can be phycogenic.
EinleitungBeim Steiûbeinteratom (ST) handelt es sich um die am häufigsten vorkommende kongenitale Neoplasie. Der Tumor besteht aus Elementen, die sich aus allen drei Keimschichten entwickeln. Die betroffenen Feten sind mit hohem Mortalitäts-bzw. Morbiditätsrisiko belastet, was auf Prämaturität, Dystokie, traumatischen Entbindungsmodus sowie intratumorale Blutung zurück-zuführen ist. Die ¾tiogenese ist nach wie vor ungeklärt, die Inzidenzrate beträgt 0,25 ± 2,28 auf 10 000 Lebendgeborene, und die Malignitätsrate wird mit 5 ± 30% angegeben. Die Prävalenz zeigt eine Verteilung zwischen männlichen und weiblichen Neonaten in einem Verhältnis von 1 : 4. Im Vergleich zur Allgemeinbevölkerung sind in diesen Fällen weitere fetale Fehlbildungen um ca. 18% häufiger anzutreffen. Das differenzialdiagnostische Spektrum reicht von der Myelomeningozele bis zum Neuroblastom. ZusammenfassungBerichtet wird über einen Fall von fetalem Steiûbeinteratom, das mittels Ultraschall in der 29. Schwangerschaftswoche festgestellt wurde. Bei der Mutter handelte es sich um eine 31-jährige III gr/II p, die sich zu spät zur Vorsorgeuntersuchung vorstellte. Sie wurde mittels Kaiserschnitt entbunden, das Kind verstarb kurz danach. Dem vorliegenden Bericht wird eine Diskussion über Diagnosestellung und Management sowie eine ausgiebige Übersicht der internationalen Literatur angeschlossen. Fazit für die Praxis: Berichtet wird über einen Fall von fetalem Steiûbeinteratom, das mittels Ultraschall in der 29. SSW festgestellt wurde. The current report refers to a case of fetal sacrococcygeal teratoma which was diagnosed ultrasonographically at 29 weeks of gestation. The 31-year old mother, III gr/II p. was often lost to follow-up until the date of admission. She was delivered by cesarean section, the newborn died shortly thereafter. Attached to the current article there is a discussion with regard to diagnosis and management and a thorough review of the international literature as well.
