Background: Discharge-planning process in hospitals is inherently complex. Information from many sources must be gathered, including patient-specific information regarding functional status and patient and family preferences. Aim of study: the study aimed to evaluate nurses' performance regarding application of discharge plan for elderly patients at Beni-Suef university hospital. Subjects and methods: A descriptive design was used to achieve the aim of the current study. The study was conducted in Beni-Suef university hospital. Simple random sample of 100 nurses worked at Beni-Suef university hospital who accepted to participate in the study. One data collection tool was used to carry out the current study namely, nurses' performance observation checklist. Results: There is a highly statistically significant relation nurses' total level of perception and their ages, qualifications, and past experience. There is a statistically significant relation nurses' total reported practice and their gender. There is positive correlation between nurses' total level of perception and their total reported practices. Conclusion and recommendations:More than three quarters of nurses had positive perception about total level of perception regarding discharge risk screening and planning. Majority of nurses had unsatisfactory practice about total reported practice regarding discharge risk screening and planning. Providing sufficient training for staff nurses that working with elderly people. Enhance self-care for elderly patients after discharge from hospital
