Gout is a potentially progressive and debilitating form of chronic inflammatory arthritis caused by the deposition of monosodium urate crystals in synovial fluid and other tissues, characterized by sudden & severe episode of pain, warmth and swelling in a joint. People suffering from recurrent attacks frequently experience pain and disability, reduced health-related quality of life and productivity and increased morbidity. NSAIDs, colchicines, glucocorticoids, uric acid lowering agent like uricostatic drugs (allopurinol, a xanthine oxidase inhibitor), Uricosuric drugs (Probenecid) are very effective but risky in patients having pre-existing renal, cardiovascular, gastrointestinal and metabolic disorders. Cupping (Al-Hijama) is a widely used therapeutic regimen of Unani system of medicine with high acceptance in Egypt and Arab countries. It is used for the treatment of various inflammatory and painful conditions like sciatica, gout, rheumatoid arthritis, pain of knee, removal of deep swelling, disease of liver and skin etc. It is a minor surgical excretory procedure related scientifically to the principles of renal glomerular filtration and abscess evacuation where a pressure-dependent excretion of causative pathological substances occurs. Cupping is thought to act mainly by increasing local blood circulation and relieving the painful muscle tension. It acts by improving microcirculation, promoting capillary endothelial cell repair, accelerating granulation, and angiogenesis in the regional tissues. It is the best deep tissue massage which normalizes the patient's functional state and progressive muscle relaxation. In Unani system of Medicine, this is a simple and economic treatment, effectively treating diseases with different etiologies and pathogenesis.
Niqras (gout) is one of the oldest known diseases and the most common form of inflammatory arthritis, resulting from the deposition of monosodium urate crystals in the joints and various other tissues, causing pain and swelling in the affected areas. The main causes of primary gout appear to be dietary and it’s thought to occur when the human body makes too much uric acid and doesn’t excrete enough of it. Since the immemorial, gout has been associated with overeating, drinking, and wealthy men who overdo it with food and drink. Overeating (ghaleez ghiza e.g. red meat, seafood), obesity, alcohol consumption, excessive coitus especially when stomach is full, indigestion were recognized as contributing factors even in ancient times. Consumption of sugar sweetened soft drinks sweet, and fructose, which had not been previously recognized, was described as a new risk factor for gout. Finally, protective factors such as skim products, high dose vitamin C and caffeinated coffee have also been described. About 70% of uric acid is produced by human body, with the remaining 30% coming from foods and beverages that contain purines. Gout is treated with medications, but a healthier diet can play an important role in treating hyperuricemia various associated diseases. A low-purine diet used to be recommended to manage gout and maintaining a healthy weight and eating a healthier diet are the best ways to treat gout. In many parts of world, there is still a tradition of using herbal drugs to combat various diseases & infections. Sibr is one of the essential components of Unani system of medicine and used by Unani physicians due to it’s a lot of medicinal properties since ancient time. It is commonly called Aloe-vera and belongs to the Lilicaceal family. It is a cactus like herb and grows in hot and arid environment. Unani physicians have been using this drug as a laxative, purgative (phlegm/bile), brain tonic, stomach tonic, liver tonic, emmenogogue, anti-inflammatory, blood purifier, antibacterial, and carminative agent. Hence, this drug having a vital place in Unani system of medicine and text. It is also use to reducing Low density Lipoprotein, increasing High density Lipoprotein, minimizing frost-bite injury, reducing blood glucose level, fighting against Acquired Immuno Deficiency Syndrome (AIDS), allergies, and boosting immune system. This article was designed to lime light the Aloe barbadensis by describing its brief toxicology, contraindications, traditional, therapeutic and others uses. Leucorrhoea (Sailan-ur-Rahem) is strictly defined as an excessive flow of a whitish, yellowish, or greenish vaginal discharge that may be normal or that may be a sign of infection. It is a common complaint particularly among Asian women. If, it is not treated in initial stage then it may convert into chronic form and leads to Pelvic inflammatory disease (PID). It is not a disease but an objective expression of a multiplicity of organic process, physiologic, pathologic and endocrinopathic. According to Unani concept Sailan-ur-Rahem is a chronic inflammatory disease, which mainly involved the mucous membrane (Gishae mukhati) of vagina. It occurs due to derangement in quality and quantity of Humors (Akhlat), especially Phlegm (Balgham) and because of accumulation of excessive morbid material (Mawad-e-Fasida) in the uterus due to weaken repulsive power (Zoaf-equwat-e-dafea). The objective of present study is to ascertain the Unani concept of Sailan-ur-Rahem as described in classical Unani literature with an aim to spread the knowledge for preventive measures and the management of Sailan-ur-Rahem by single and compound Unani drug formulations, which are affordable, commonly available and approximate free of side effects.
