Purpose To analyze the prescribing trends over a 7-years period, between 2013 and 2020, in a tertiary hospital (Hospital Clinico San Carlos, Madrid, Spain) and its health area. Material and methods A retrospective study on the data collected from the information systems, “farm@web” and “Farmadrid”, of glaucoma prescriptions in the framework of a public health system (Spanish National Health System) during the last seven years. Results Prostaglandin analogues were the most commonly used drugs in monotherapy during the study period (range: 36.82% − 47.07%). Fixed combinations of topical hypotensives had an upward trend since 2013 (range: 39.99% − 54.21%), becoming the most dispensed drugs in 2020 (48.99%). Preservative-free eye drops (lacking benzalkonium chloride, BAK) have displaced preservative containing topical treatments in all pharmacological groups. In 2013, BAK-preserved eye drops accounted for 91.1% of the total prescriptions, however in 2020 they only accounted for 34.2% of total prescriptions. Conclusions The results of the present study highlight the current trend to avoid BAK-preserved eye drops for the treatment of glaucoma.
To report the clinical results on the use of corneas frozen in Eusol-C as tectonic corneal grafts.Retrospective review of medical records of patients who received frozen corneas as emergency tectonic grafts from 2013 to 2020. Corneas had been stored in Eusol-C preservation media at − 78 °C for a mean time of 6.9 months. Diagnosis, transplant characteristics, microbial culture results, anatomic integrity, epithelial healing, neovascularization, transparency, infection and need for additional surgeries were registered. Fifty corneas were used in 40 patients (mean age 60.5 years, 20 males) with a median follow-up of 27.3 months after surgery. Need for tectonic graft was due to: perforation secondary to immune diseases (6, 12%), neurotrophic ulcer (11, 22%), trauma (3, 6%), corneal infection (11, 22%), chronic disorders of the ocular surface (9, 18%) and previous corneal graft failure (10, 22%). Mean size of grafts was 5.6 mm and 36 cases (72%) also received an amniotic membrane graft. Thirty-eight corneas achieved epithelization (76%), 25 (50%) were clear and 19 (38%) developed neovascularization. None of the corneas were rejected. Seventeen corneas (34%) failed: 7 (14%) due to reactivation of baseline disease and 10 (20%) due to primary graft failure. Four corneas (8%) had positive microbial cultures suggestive of contamination and 2 (4%) developed a cornea abscess non-related to a positive microbial culture. Long-term preservation of donor corneas in Eusol-C at − 78 °C is a viable technique to meet the needs of emergency grafts with minimal equipment.
Background: The aim was to determine visual outcomes and patient satisfaction in patients undergoing cataract surgery after the binocular implant of multifocal difractive Intensity IOL Methods: 21 patients were evaluated. Six weeks after surgery, uncorrected distance visual acuity(UDVA), corrected distance visual acuity(CDVA), distance corrected intermediate visual acuity at 60 cm(DCIVA) and distance corrected near visual acuity at 40cm (DCNVA) were determined using the ETDRS test. Defocus curves were produced both in photopic and mesopic conditions. Contrast sensitivity(CSF) was measured using the CSV-1000 test. Patients were shown pictures about dysphotopic phenomena and informed about their meaning with a likert scale from 0 (no problem) to 4 (overwhelming). Results: Post implantation mean logMAR Binocular UDVA, CDVA, DCIVA and DCNVA were 0.07± 0.09, -0.01 ± 0.04, 0.08 ± 0.05 and 0.12 ± 0.06 respectively. Photopic defocus curve showed a extended range of good vision. Mesopic defocus curve results were better than previously reported with trifocal designs. Mean binocular CSF values for 4 spatial frequencies (3, 6, 12 and 18 cpd) were 1.55±0.29, 1.60±0.17, 1.29±0.26 and 0.81±0.15 log. units, respectively. Halos were more frequent than starburst and glare with a Likert scale mean value of 0.86±0.83. Conclusions: The IOL provided a continuous range of vision from distance to near. Patients were not bothered or only slightly bothered in relation to the visual disturbances.
