In a series of 112 patients who had undergone a duodenopancreatectomy (48 carcinoma, 64 pancreatitis), there were 2 deaths (1 carcinoma, 1 pancreatitis). Including a continuous series of 95 cases without death, there were no failures observed in the anastomotic technique described. The fatal risk of a pancreatic leak can be avoided by broad plication with a Roux loop and integrated dorsal ductanastomosis. The function of duct anastomosis was proved to be patent by the presence of chymotrypsin in the stool of all 26 patients examined.

A total of 341 large-bowel resections are reported that were carried out between 1 January 1984 and 31 August 1984. In 261 cases, there was a large-bowel carcinom, most of them in the sigmoid (33%) and rectum (29%). Patients were prepared without orthograde lavage by 2 × 75 ml X-Prep 2 days prior to the operation, 75 ml X-Prep the day before surgery, and by an enema in the evening. Using this very effective procedure, we performed one-stage resections in a series of 201 cases with only 1 lethal anastomosis insufficiency; 3 other cases developed an insufficiency that was not evident clinically. All patients were treated by perioperative antibiotic prophylaxis (cefotiam, gentamycin, metronidazole). The anastomoses were performed either by extramucous one-layer suture using absorbable material or by causing an EEA stapler.Zusammenfassung. Unter Verzicht auf orthograde Darmsptilung erfolgte die Darmvorbereitung dutch 2 × 75 ml X-Prep am 2. pr~operativen Tag, am n~tchsten Tag 1 X-Prep und abends hoher Einlauf, gleichzeitig Ende der oralen Ern~hrung, Trinken war in unbegrenzter Menge erlaubt, bei alten und katabolen Patienten gleichzeitig Beginn der parenteralen Ernahrung. Obligate perioperative Kurzzeit-Anfibioticaprophylaxe. Nahttechnik: Extramuc6se, einreihige StoB-auf-StoB-Naht mit resorbierbarem Nahtmaterial oder EEA-Stapler.
