Background: Men who have sex with men (MSM), particularly those with HIV infection, are at high risk for contracting syphilis. However, little is known about the effect of syphilis on HIV-1 RNA and CD4 count among HIV infected MSM. Methods: MSM were enrolled into the MSM-VCT study cohort at Sanglah Hospital in Bali between June 22 nd 2011 to February 13 rd 2012. All participants had anti-HIV test and VDRL test at baseline, with TPHA confirmation for reactive VDRL results. Plasma HIV-1 RNA levels and CD4 counts were measured at baseline and month 6 visits. T-test or non-parametric Mann-Whitney test were used to compare changes in CD4 counts and HIV-1 RNA levels between those with and without syphilis. To analyze difference of baseline and 6 month visit in each group, we used paired students t-test.
Background: Psoriasis is a complex, multifactorial, inflammatory, chronic autoimmune disorder that changes the life cycle of skin cells. In countries around the world, the prevalence of psoriasis ranges from 0.09% to 11.43%. Objective: This study aims to determine the prevalence and clinical manifestations of patients with psoriasis in RSUP Sanglah Denpasar from the year 2017 to 2018. Method: The study used is a descriptive retrospective study design using total sampling with142 samples recorded. Results: The prevalence of psoriasis was 0.603% in 2017 and 0.708% in 2018. Between 2017 till 2018, a total of 142 patients reported having psoriasis with 73 patients (51.4%) in 2017 and 69 patients (48.6%) in 2018. The cases were dominated by males with 90 patients (63.4%) and females with 52 (36.6%) for both the years. Psoriasis patients between the ages of 36-45 years were reported the most with a proportion of 23.2%. The least number of patients were reported below the age of 5 years. There was a total of 63(44.4%) patients diagnosed with psoriasis vulgaris which is also the most common type of psoriasis case found in Sanglah. The least number of patients were psoriasis inverse and erythrodermic with 4(2.8%) patients each. Conclusion: The clinical manifestations used to diagnose psoriasis were red sharp boundaries erythematous plaque, covered with silver or scaly white squamae and geographic skin lesion. This clinical finding was used for almost all psoriasis diagnosis.
Introduction: The mechanism of nerve damage in ENL patients is important to be revealed because the problem of nerve damage is not getting enough attention in handling ENL patients. Some pro-inflammatory mediators that have been studied are risk factors for nerve damage in ENL.Aim: This study aim in knowing the mechanism of nerve damage in the relation with its risk factors, which are CD64 neutrophil, IL-17, MMP-9, and iNOS with PGE-2 and IL-10 protective factor and IL-10 in ENL patients.Method: This is an analytic cross-sectional design study with tissue samples from 30 leprosy patients with ENL in Dr. Soetomo General Hospital Surabaya. Samples from biopsy examined using IHC method to evaluate S-100, IL-10, IL-17, PGE-2, iNOS, MMP-9, and CD64 neutrophil expressions. The result calculated within 20 view field with 1000 times magnification.Result: CD64 neutrophil (p=0.022, r=-0.417), IL-17 (p=0.001, r=-0.560), iNOS (p=0.005, r=-0.502) and MMP-9 (p=0.025, r=-0.409) has significant correlation with S-100. There is no significant correlation between IL-10 (p=0.061) and PGE-2 (p=0.407) with S-100.Conclusion: Expressions of IL-17, CD64 neutrophil, MMP-9, and iNOS in tissue correlate with nerve damage in ENL patients.
Background: Syphilis is an infectious disease caused by Treponema pallidum and is transmitted primarily through sexual intercourse. However, late latent syphilis accompanied by ocular syphilis in geriatric patients is quite seldom. This study aims to determine a case report regarding late latent syphilis accompanied by ocular syphilis in a geriatric patient.Case Description: A 75 years old man, was consulted from the Ophthalmology Department to the Dermatology and Venereology Department with suspect syphilis. Patient came with a chief complaint of blurred vision in the right eye since 2 weeks ago, which is getting worse since 4 days ago. The laboratory results of complete blood routine, renal function tests, liver function tests, blood sugar levels are within normal limits. The titer result of VDRL was 1:16 and TPHA was 1: 160. Based on anamnesis, physical and laboratory examination, the patient was diagnosed with late latent syphilis and ocular syphilis. The patient was given three weekly interval dose of Benzathine Penicillin G 2.4 million international units intramuscularly.Conclusion: One-month follow-up after the treatment, the patient's condition improved. Blurred vision in the right eye was reduced. There are no complaints of headache and hearing loss.
