This study confirms that early discharge within 48-72 h in selected low risk patients after STEMI treated with successful primary PCI is feasible and safe, with outcomes comparable to the later discharge. This strategy applies to more than a quarter of all STEMI patients.
V uplynulých dekádách bylo možné sledovat jasný trend ke zkracování pobytu v nemocnici u pacientů s infarktem myokardu s elevacemi úseku ST (STEMI). Aktuální doporučení Evropské kardiologické společnosti pro léčbu STEMI uvádějí, že u vybraných pacientů může být zvážena časná dimise (po přibližně 72 hodinách), pokud je zajištěno adekvátní navazující sledování. Autoři prezentují prospektivní analýz u 25 nízce rizikových pacientů se STEMI, léčených úspěšnou primární perkutánní koronární intervencí (PCI) a propuštěných v průběhu 48-72 hodin od přijetí. Během 30denního sledování se vyskytla pouze jedna neplánovaná hospitalizace z nekardiální příčiny, nebyly pozorovány žádné jiné závažné komplikace. Uvedená data demonstrují, že časná dimise po STEMI u vybraných, nízce rizikových nemocných je s ohledem na podmínky běžné klinické praxe možná a bezpečná. Tuto strategii lze uplatnit u nejméně 14 % nemocných s nízkým rizikem následných komplikací.
Ecologically significant elements are essential not only for the development and quality of life in rural areas, but also for agricultural management. Services provided by these ecosystems ensure water retention in the landscape, a protection against erosion and an increase in biodiversity. In the context of current political debates, the importance and necessity of highlighting the value of ecologically significant elements is desirable from the farmer's point of view. This study inquires into costs and benefits while implementing a selected ecologically significant element -landscaping orchard in an agricultural area, taking into account not only the explicit benefits that could be marketed, but also the benefits floating from the ecosystem services. The case study found out that whilst complying with the general conditions to be able to apply for the State subsidies with the value of ecosystem services, the Net Present Value is positive for the given project. The monetary value of ecosystem services provided by the given landscaping orchard is estimated to be, according to Ecosystem Services Valuation Database (2020), 125 thous. CZK.year -1 per 1.6 ha. That is a considerable amount which should be noted not only by the farmers but also by the policy makers who should reconsider the subsidies for farmers and pay more attention to their financial needs.
Problem with soil erosion has not yet been satisfactorily solved with a holistic approach through a precisely targeted and duly explained strategic plan in the Czech Republic. Even though there are effective means to undertake needed measures, conservation grassland being one of them. The research aims to contribute to the debate that concerns battling soil erosion in Czech conditions. The idea of conservation grassland as an effective tool to battle soil erosion is well-known by the experts and farmers alike. Although there has been a discussion on what should be the extent of conservation grassland on a country-level in Czech conditions, there still has not been enough vigour to push the idea forth. We set two research questions.Firstly, what should be the total volume of agricultural land that should undergo the conservation grassland. Secondly, how to adjust subsidy scheme for making the conservation grassland as erosion protection economically viable for the farmers. We provide answers with holistic analysis of all Land Parcel Blocks (LPB) in 2016-2021 period where the erosion events were analysed, and the Erosion Hazard Classes (TEO) were considered too. We concluded that if we take into the account a five-year project according to 2023-2027 Common Agricultural Policy scheme, more than CZK 3.4 billion worth of soil could be saved if 103,428.4 hectares were grassed. For a conventional farmer, additional subsidy scheme would have to be presented to compensate farmer's expenses for implementing the conservation grassland that secures an anti-erosion ecosystem service and other ecosystem services as well.
