APC and PDT are equally effective in eradicating Barrett's mucosa, with PDT being the more expensive treatment. However, PDT is more effective in eradicating dysplasia and the extra benefits of PDT are generated at an extra cost. The occurrence of buried columnar glands and carcinoma warrants caution. Long-term follow-up is needed to assess cancer prevention and the durability of the neo-squamous epithelium to justify these interventions.
BackgroundHealth related quality of life (HRQL) has been recognised as an important paediatric outcome measurement. One of the more promising measures to emerge in recent years is the Pediatric Quality Of Life Inventory (PedsQL™), developed in the US. Advantages of the PedsQL™ include brevity, availability of age appropriate versions and parallel forms for child and parent. This study developed a UK-English version of PedsQL™ generic module and assessed its performance in a group of UK children and their parents.MethodsPedsQL™ was translated to UK-English. The psychometric properties of the UK version were then tested following administration to 1399 children and 970 of their parents. The sample included healthy children, children diagnosed with asthma, diabetes or inflammatory bowel disease and children in remission from cancer.ResultsPsychometric properties were similar to those reported for the original PedsQL™. Internal reliability exceeded 0.70 for all proxy and self-report sub-scales. Discriminant validity was established for proxy and self-report with higher HRQL being reported for healthy children than those with health problems. Sex differences were noted on the emotional functioning subscale, with females reporting lower HRQL than males. Proxy and self-report correlation was higher for children with health problems than for healthy children.ConclusionThe UK-English version of PedsQL™ performed as well as the original PedsQL™ and is recommended for assessment of paediatric HRQL in the UK.
Methods: Frontline radiographers, from seven regional hospitals, who performed duties in general radiography, were provided with a workplace violence questionnaire. General radiography refers to plain film X-ray services in general rooms (including out patient clinics), A&E and portable services on wards. Materials relating to workplace violence, for example guidelines and training information, were provided by hospital managers. Results: Out of 281 questionnaires, 150 were returned (response rate of 53%). Sixty-one percent of radiographers had experienced violence in the past 3 yr and 34% of victims had encountered incidents more than 5 times. From respondents who had experienced abuse, verbal abuse (97%) was most frequently reported, and the predominant source of violence was patients (p<0.0001). Respondents identified long waiting times, communication issues and understaffing as key risk factors. The Accident & Emergency Department was the highest risk area (p<0.0001). Almost two thirds (65.91%) of radiographers who experienced verbal abuse ignored events. Although no severe injury was reported, indirect impact, including increased work stress, job dissatisfaction, depression and increased sick leave, were highlighted as negative consequences of violence. 77% of respondents felt that support from departments was inadequate and only 11% had attended courses on prevention of Field Study occupational violence. Conclusions: Workplace violence is a critical problem in Hong Kong. Further research is recommended to investigate the problem.
