Among 4,000 cases of acute head injuries for the last 3 years, the most serious type (cerebral lacerations, intracerebral hematomas) were reviewed and those treatment and prognosis were discussed. In 17 cases of acute head injuries (type IV), 8 were died postoperatively, in which all of subdural hematomas were died. In 110 cases of acute head injuries (type III) 26 were died, most of which were died shortly after the injuries. As to their prognosis, from the point of deapth of consciousness, those who elapse into frank coma immediately following the injuries show poor prognosis, those who are in semi-comatose, stuporous or delirious show good prognosis. Grade in disturbance of consciousness is not always parallel to post-traumatic syndrome. Furthermore, we discussed about operative indication, as a conservative management, artificial hibernation with hypothermia is most effective for acute head injuries.
