Soft tissue sarcomas arise infrequently and make up less than 1% of all cancers. An undifferentiated pleomorphic sarcoma (UPS), formerly known as a malignant fibrous histiocytoma, is a malignant subtype of soft tissue sarcoma identified by a lack of specific immunohistochemical markers for lineage differentiation. These tumors are aggressive and enlarge rapidly with an increased risk of metastasis, thus prompt diagnosis, treatment, and post-resection surveillance are imperative. This case report demonstrates an incidental finding of a large undifferentiated pleomorphic sarcoma of the posterior thigh in an 86-year-old male initially evaluated with a bedside ultrasound.
The project is to build a full-scale working replica of the Twin Coil dialysis device designed by Willem Kolff who introduced this much improved dialysis machine for hospital and clinical use in 1955. The first step was to analyze original models of the Kolff Twin Coil on display in Washington, DC and in Largo, Florida and understand the design and engineering aspects of the device and how the Twin Coil design promoted effectiveness and efficiency in dialysis. When the design aspects of the Twin Coil were understood the task was to build a functional replica of the electrical circuitry and the control panel and interface with the three motor pumps. The approach to this was to study and understand the workings of the panel, acquire modern counterparts fitting to the replica's demand, and assemble the finished product. The finished panel and interface will be assembled and connected to the dialysis drum, built by Temple University, creating a final working replica Twin Coil dialysis machine.
Point of Care Ultrasound (POCUS) in the Emergency Department (ED) is a rapidly evolving field and has improved time to diagnose and management. POCUS of the bowel is still a novel concept for many departments. However, there is a potential application for POCUS to augment or replace traditional diagnostic imaging of small bowel obstruction (SBO). This is a case of SBO detected on POCUS in just a few minutes, which raises the possibility of POCUS improving time to surgical consultation and ED disposition in the future.
