Describimos el caso de una paciente de 35 años, previamente sana y sin ningún factor de riesgo para el desarrollo de rabdomiólisis. Se presentó a sala de urgencias por la tríada clásica de rabdomiólisis posterior a la vacuna COVID-19 con ChA-dOx1. La base del tratamiento fue hidratación parenteral, estrecha vigilancia de los niveles de creatina fosfocinasa y de la función renal, con el objetivo de evitar la instauración de lesión renal aguda. Evolucionó a remisión total de sintomatología, sin presentar mayores complicaciones.
Female patient admitted with hypotension (60/40 mmHg) secondary to septic shock of urinary origin, which was refractory to treatment with crystalloids; initiating vasopressor support with norepinephrine (NE) at response doses without improvement of the picture, presenting peripheral digital ischemia induced by vasopressor. Of the patients admitted to the emergency department with clinical and biochemical data of septic shock, 46% need one or more vasopressors, with NE being the most used, to maintain a stable hemodynamic state. In most patients, it is necessary to rule out other entities such as autoimmune diseases and systemic lupus erythematosus.
