Emphysematous vaginitis represents a rare and self‐limited condition, which presents with vaginal discharge. Despite its benignity, it has some worrisome imaging features, which should be recognized in order to avoid unnecessary invasive procedures.

Teaching Point: Lymphomatous involvement of the skeletal muscle is rare; however, because lymphoma can be treated with chemotherapy, it is crucial to consider it in the differential diagnosis of a soft tissue mass in the appropriate clinical and imaging setting.

Teaching Point: An ureteroinguinal hernia is a very rare diagnosis with typical radiological findings that must be reported, particularly in surgical candidates.
