Bilateral deep vein thrombosis (DVT) should prompt investigation for pro-thrombotic conditions and the exclusion of vascular compromise due to intrinsic and extrinsic factors. The authors present the case of a 47-year-old man admitted with bilateral DVT and diagnosed with inferior vena cava (IVC) infra-renal segment agenesis, and discuss the management of this rare condition. LEARNING POINTS • Bilateral deep vein thrombosis in young and otherwise healthy individuals should prompt investigation of the aetiology and risk factors in case lifelong treatment and anticoagulation therapy is required. • Although rare, anatomical variations and congenital malformations should be considered even in adult patients. • Rare medical cases should prompt a multidisciplinary approach to investigation and treatment, as prognosis will depend on both therapeutic procedures and anticipation of complications. Metabolic myopathies are a diverse group of rare genetic disorders associated with recurrent episodes of rhabdomyolysis, induced by triggers such as fever or exercise. In these disorders, the energetic metabolism is compromised resulting in damage of the muscle cells. The diagnosis can be challenging but is essential for the correct treatment. Carnitine palmitoytransferase II (CPT-II) deficiency is the most common long-chain fatty acid oxidation defect, with the adulthood form requiring additional external triggers. The authors present a case of a young-male adult with recurrent episodes of rhabdomyolysis, one of them presented with acute renal failure and acute hepatitis. The diagnostic is demanding, which requires a high level of suspicion. The adequate treatment of these patients improves the muscle function and prevents other episodes of severe rhabdomyolysis.
O objetivo deste manuscrito é mostrar a importância de alguns fatores-chave para que os clusters marítimos (indústria naval) se tornem inovadores e competitivos. Essa pesquisa tem objetivo exploratório, com análise qualitativa e descritiva dos dados. Foi baseado em estudos de artigos e uma inspeção crítica detalhada em publicações de órgãos oficiais com a intenção de enumerar os fatores comuns aos clusters que os propiciaram ser inovadores. Com esta pesquisa, constatou-se que os clusters estudados possuem fatores-chave comuns como infraestrutura adequada, rivalidade entre firmas, sofisticação do comprador, conhecimento codificado e tácito, entre outros. Todos estes são estudados no Modelo Diamante de Porter - DFM. Embora esse estudo tenha sido direcionado para clusters relacionados a indústria naval, a semelhança das reações operacionais dos vários clusters industriais leva a crer que estes resultados também possam ser úteis para outros tipos de indústria. Este manuscrito demonstra a importância do cumprimento de alguns objetivos para a obtenção da competitividade sustentável com base na inovação, de clusters industriais.
