Acne fulminans (AF) is a severe form of inflammatory acne, characterized by an abrupt explosive flare of multiple ulceronecrotic lesions, which can be associated with systemic symptoms. 1 We report the first case of therapy-refractory AF successfully treated with apremilast, and discuss the benefits of this medication.A 15-year-old-adolescent with personal history of overweight and acne conglobata was started on isotretinoin 0.4 mg/kg/day. Six weeks later, he presented at the emergency department with a sudden worsening of his acne with numerous painful papules, pustules, and hemorrhagic nodules with crusts, widely distributed over his back and chest (Figure 1A).
