Polycystic ovary syndrome is currently regarded as a multifactorial disease with a genetic predisposition, characterized by a wide variability in clinical manifestations that affect the reproductive, endocrine and metabolic functions of women. Despite the great scientific interest in this pathology, the causes and mechanisms of development remain not fully understood. In this article, we reviewed the most current research on the genetic markers of PCOS.

■ Актуальность. Несмотря на широкое внедрение в развитых странах мира оперативных вагинальных родов с помощью вакуум-экстракции плода (ВЭП), этот метод остается недостаточно популярным в нашей стране, что связано с необоснованным мнением о высоком травматизме для матери и плода. Однако в связи с усовершен-ствованием моделей и очевидными преимуществами в простоте и удобстве его использования применение этой родоразрешающей операции становится реальностью. Цель исследования. Оценка влияния оперативных влага-лищных родов путем вакуум-экстракции плода на состояние родильниц и новорожденных. ■ Actuality. Although in the developed countries of the surgical vaginal delivery by a vacuum extraction of the fetus the widespread introduction, this method is not sufficiently popular in our country, due to the unfounded opinion of the highest injury to the mother and fetus. However, due to the improvement of models and the obvious advantages of simplicity and ease of use, the application of this operation becomes reality. Aim. The assessment of the surgical vaginal delivery by a vacuum extraction of the fetus on the shape of parturient women and newborns. Materials and methods. Made a retrospective analysis of the outcome of the 192 vaginal operative deliveries by using a single use vacuum system KIWI according to the labor histories and stories of newborns for 2014-2015 in regional perinatal center, Kursk. We studied the anamnestic data and the results of monitoring the pregnancy, childbirth, postpartum and newborn state. The calculations were made using the package of standard programs. Results. In the majority of cases the postpartum period was uneventful, the cases of severe obstetric injury was not revealed. 80.21% of the children were recovered in a satisfactiry condition, although acute fetal hypoxia was observed in 90.1% of cases, suggesting the efficacy of using the vacuum extraction. clinically significant lesions, such as subgaleal hematoma, skull fracture are not registered. Conclusions. Analyzing the consequences of this method, we can confidently say that the operation of vacuum extraction with timely and correct use of a technical execution has no negative impact on the organisms of mother and child and is justified by positive outcomes in emergency situations.■ Keywords: surgical delivery; vacuum extraction of the fetus; cesarean section; forceps; fetal hypoxia.
Aim. To analyze the efficiency of the use of inosit in the treatment of polycystic ovary syndrome (PCOS) in women of reproductive age.Materials and methods. The prospective study included 80 patients aged 21 to 35 years with a diagnosis of PCOS. The main group consisted of 45 women who were prescribed inosit and lifestyle modification to correct symptoms of PCOS. The control group consisted of 35 women for whom was recommended only lifestyle modification. The condition of the patients was assessed at the beginning of therapy, after 3 and 6 months.Results. During therapy, the patients of the main group showed a statistically significant effect of inosit on metabolic status: a decrease in fasting glucose and insulin levels, body mass index; hormonal status: a decrease in the level of androstenedione, testosterone, LH, normalization of the LH/FSH, a decrease in the clinical manifestations of hyperandrogenism; restored menstrual cycle with ovulationConclusions. The use of inosit along with a balanced diet and physical activity is an effective means for correcting clinical manifestations and improving the quality of life of women with PCOS.
