Mastectomy is considered as the standard therapy for gestational breast cancer. Since radiation therapy is harmful for the fetus, conservative surgery is rarely used during pregnancy. Among 16 patients with gestational breast cancer, 10 and 6 were treated with conservative surgery and mastectomy, respectively. No local recurrences occurred with a median follow-up time of 87 months. Among the 10 patients treated with conservative surgery, 3 chose therapeutic abortion and 7 opted to continue their pregnancy. Concerning these 7 fetuses, there were no congenital anomalies, nor growth restriction. All children were normal physically and neurologically. We concluded that conservative breast surgery may be an alternative to mastectomy in the treatment of gestational breast cancer and is safe for the fetus.
Chez les femmes jeunes, les cancers du sein néces-sitent souvent des thérapeutiques modifiant les hormones sexuelles. Les conséquences en sont la prise de poids, les bouffées vasomotrices, le vieillissement cutané, la sécheresse vaginale et les modifications de l'humeur conduisant à des troubles de la sexualité, une transformation du schéma corporel et des modifications de la relation avec l'entourage. Trop souvent négligés, ces effets secondaires devraient être expliqués aux patientes et traités afin d'en réduire leurs répercussions somatiques et psychologiques.
Abstract:Breast cancers occurring in young women often need adjuvant therapeutics leading to hormonal variations. Side effects are numerous and may include weight gain, vasomotor symptoms, ageing of the skin, vaginal dryness and mood changes, leading to sexual dysfunction, modification of body representation and familial relationship alteration. Physicians should diagnose, explain and treat these symptoms with the aim to decrease the somatic and psychological impact.
