Dear Editor, Hydroxychloroquine (HCQ) causes drug eruptions, including psoriatic diseases. 1 We report a case of annular pustular psoriasis (APP)/ recurrent circinate erythematous psoriasis (RCEP) induced by HCQ in a patient with a gain-of-function mutation in caspase recruitment domain family member 14 (CARD14).A 30-year-old Japanese woman, who was diagnosed with systemic lupus erythematosus (SLE), received oral prednisolone (PSL) (30 mg/day) and HCQ (200 mg/day). She had anti-dsDNA and anti-SSB antibodies. Her symptoms of SLE improved and she had no serological abnormalities reflecting disease activity. However, edematous erythema developed on the entire body when PSL was tapered to 17.5 mg/day (Figure 1a,b). Skin biopsy revealed interface dermatitis and the lupus band test was negative.
