We report the case of a patient where loss of capture by a pacemaker occurred when they were placed in the lateral position for hip surgery. Cardiac output was lost but capture and output were restored on turning the patient back into the supine position for chest compressions. We hypothesise either a pacing wire displacement or possibly a new ischaemic event as the cause. This appears to have been a rare complication in a patient with an established pacemaker.
