ObjectivesTo determine whether febuxostat with stepwise dose increase is as useful as colchicine prophylaxis in reducing gout flares during the initial introduction of urate-lowering therapy in patients with gout in comparison with febuxostat with no dose titration.MethodsIn this prospective, multicentre, randomised open-label comparative study, patients were randomised to group A (stepwise dose increase of febuxostat from 10 to 40 mg/day), group B (fixed-dose febuxostat 40 mg/day plus colchicine 0.5 mg/day) or group C (fixed-dose febuxostat 40 mg/day) and observed for 12 weeks. Gout flare was defined as non-steroidal anti-inflammatory drug use for gout symptoms.ResultsA total of 255 patients were randomised, and 241 patients were treated. Among the treated patients, gout flares were experienced by 20/96 (20.8%) in group A, 18/95 (18.9%) in group B and 18/50 (36.0%) in group C. The incidence of flare was significantly lower in groups A and B than that in group C (P=0.047 and P=0.024, respectively), although the differences were not significant after correction for multiple comparisons. No significant difference was noted between the incidence of gout flare in groups A and B.ConclusionsOur data suggested that stepwise dose increase of febuxostat and low-dose colchicine prophylaxis effectively reduced gout flares in comparison with fixed-dose febuxostat alone. Stepwise dose increase of febuxostat may be an effective alternative to low-dose colchicine prophylaxis during the introduction of urate-lowering therapy.Trial registration numberUMIN 000008414.

Solutions of CPPU and BA were applied to ovaries of melon (Cucumis melo) flowers with or without pollination, and the effects on fruit set, growth, and sugar content were investigated. Treatment with CPPU increased fruit set in both seeded and seedless melons. Even at low concentrations, CPPU had a strong effect on fruit set in the seeded melons. In seedless melons, CPPU induced 100% parthenocarpic fruit set when applied with 10 mg·L–1; lower concentrations were much less effective. Treatment with BA increased fruit set in seeded melons, but was not particularly effective in the absence of pollination. During the first 10 days after anthesis, CPPU promoted fruit growth, but between 8 and 13 days after anthesis, the growth rate was lower than in the controls. Treatment with CPPU had little effect upon soluble solids (SS) levels in seeded fruit. SS content was significantly lower in seedless than in seeded fruit; this difference was larger in the placenta than in the mesocarp. Sucrose levels of both seeded and seedless fruits were consistently higher than glucose and fructose levels. High concentrations of CPPU reduced sucrose levels in the placenta of seedless fruit. These results indicate that seeds play an important role in sugar accumulation and melon fruit growth during later stages of development. SUMMARY The purpose of this investigation was to investigate the rheological properties of four commercially available gutta perchas for root canal filling. The relaxation modulus [Gr(0): instantaneous shear modulus] and specific volume of their materials were examined. In addition, the quantity of heat was also studied by differential scanning calorimeter. In a lower temperature range than the first-order transition temperature (melting point), the Gr(0) values of each material were almost identical. A marked decrease of Gr(0) was observed at the melting point, and the range of the first-order transition temperature at heating was from 42 AE 0 to 60AE0°C. At higher temperatures than the first-order transition temperature of each material, a considerable difference in Gr(0) values was observed. The transition temperatures obtained by the results of the Gr(0), specific volume and quantity of heat agreed with one another. A marked specific volume change was observed at the first-order transition temperature. The technique using melted gutta percha may not be favourable compared with the conventional lateral condensation technique because melted gutta percha undergoes a large amount of shrinkage during setting.
